右美托咪定在颈椎手术经鼻纤维支气管镜清醒插管中的麻醉效果

摘　　要：目的观察右美托咪定在颈椎手术经鼻纤维支气管镜(纤支镜)清醒插管中的应用效果。方法选择颈椎手术患者60例,随机分成Y组(右美托咪定组)和对照组S组(生理盐水组),分别观察并记录其在诱导插管前,插管中及插管后的MAP、HR、SPO_2、BIS及Ramsay评分的变化,统计两组的插管成功率、插管时间及两次以上插管成功例数。结果 Y组各参数均变化不大,S组变化大(P<0.05),插管成功率Y组明显高于S组(P<0.05)。结论右美托咪定镇静镇痛效果确切,循环稳定,呼吸抑制轻,能提供满意的插管条件及耐受性,在颈椎手术清醒纤维支气管镜插管中应用良好。Objective To observe the efficacy of dexmedetomidine during cervical vertebra surgery by nasal intubation under fiberoptic bronchoscope on awake patients. Methods The 60 patients subjected to cervical vertebra surgery were enrolled in this study,and evenly allocated to the Y group （dexmedetomidine group） and S group （NS group） at random;the results of MAP,HR,SPO2,BIS and the score of Ramsay before intubation, intuhating,and after intubation,as well the success rate of intubation,intubation time and the number of intubation which need more than two times were observed and recorded. Results The changes of Y group were small while a significant increase were observed in S group （P〈0.05）. Compared with the S group,Y group had higher success rate of intubatian,shorter time of intubation and less need more than two times intubation. Concision Dexmedetomidine demonstrates excellent analgesic effects,facilitates endotracheal intubation,improves tolerance,while maintaining respiratory and hemodynamic stability for patients operation for cervical vertebra.
