摘  要:目的探讨320排容积CT 5%和10%R-R间期间隔重组定量评估左心室功能的差异性和临床应用价值。方法 22例健康体检者(无心脏疾病)行320排CT常规冠状动脉成像,分别以5%、10%R-R间期间隔重组全心周期图像,运用心功能软件分别计算两种间期间隔左心室舒张末期容积(EDV)、收缩末期容积(ESV)、每搏输出量(SV)、左心室射血分数(EF)。以二维超声心动图(2DE)的结果作为参考,采用配对t检验和Pearson相关分析比较两种重组方法的差异性和相关性。同时计算两种间期间隔重组心功能分析所用总时间。结果采用5%、10%R-R间期间隔重组心功能各项指标测量值比较差异无统计学意义(P>0.05),5%R-R间期间隔EDV、ESV、SV、EF均略高于10%R-R间期间隔,两者之间具有高度相关性(r=0.853~0.976,P<0.001)。320排CT 5%、10%R-R间期间隔重组分别测量左心功能与2DE之间差异无统计学意义(P*>0.05)。5%、10%R-R间期间隔重组心功能分析平均花费时间分别为(26.2±2.3)min和(14.7±1.6)min。结论 320排CT行冠状动脉检查的同时可以评估左心室功能,5%和10%R-R间期间隔重组图像均能提供准确的数据,但以10%R-R间期间隔重组图像评估左心功能相对方便、快捷。Objective To evaluate the difference and clinical application of Left ventricular( LV) function of 320-detector computed tomography between every 5% and 10% R-R interval reconstruction. Methods Twenty two healthy adults underwent 320-detector CT coronary angiography. LV end-diastolic volume( EDV),end-systolic volume( ESV),stroke volume( SV),ejection fraction( EF) were measured from original data sets using reconstruction at every 5% and 10% R-R interval. These LV function parameters were compared with 2D-echocardiography( 2D-E),being used as the reference standard. The total time for LV function analysis was checked. Results There was no significant difference in EDV,ESV,SV,EF between every 5% and 10% R-R interval Reconstruction and showed good correlation( r = 0. 853 ~ 0. 976,all P〈 0. 001). LV function parameters( EDV,ESV,SV,EF) measured with every 5% R-R interval reconstruction were slightly higher than that measured with every 10% R-R interval. There was no significant difference in LV function parameters between every 5% and 10% R-R interval reconstruction with 320-detector CT and 2DE( P*〉 0. 05). The mean time for LV function analysis in every 5% and 10% R-R interval reconstruction were( 26. 2 ± 2. 3) min and( 14. 7 ± 1. 6) min. Conclusion 320-detector CT coronary angiography can evaluate coronary artery and LV function. Both every 5% and 10% RR interval reconstruction are sufficient to evaluate LV function,but 10% R-R interval is more time effective.
