某院2015年1—6月用药不适宜医嘱分析

摘　　要：目的对该院用药医嘱的点评结果进行分析,为临床合理用药提供参考。方法采用回顾性调查方式,利用随机数字表法抽取该院2015年1—6月临床用药医嘱1 590条,对不合理医嘱进行分析和总结。结果不适宜用药医嘱共396条,占总医嘱数的24.91%,其中妇产科不适宜医嘱发生频率最高。不适宜用药类型主要表现为用法或用量不适宜、有配伍禁忌或者不良相互作用、药品剂型或给药途径不适宜。结论该院临床用药及监控管理制度仍然存在缺陷,应加强完善临床用药医嘱审核制度,提升合理用药水平。Objective Toprovide reference for clinical rational use of drugs by analyzing medical orders evaluation in our hospital. Meth-ods Byretrospective survey,1159 clinical medical orders from January to June 2015 were selected randomly in Department of Pharmacy, Huangshi Central Hospital of Edong Healthcare Group. The irrational medical orders were analyzed and summarized. Results There were 396 irrational medication orders,accounting for 24. 91% . The irrational medical order ratewas the highest in gynecology and obstetrics de-partment. The irrational drug use mainly manifested itself asinappropriate usage and dosage,incompatibility or adverse interactions and in-appropriatedrug dosage form or route of administration. Conclusions The clinical drug use and monitoring management system arestill flawed in our hospital. We should strengthen and improve the clinical medical order review systemto promote the rational drug use.
