T-淋巴细胞酶联免疫斑点法检测在结核性结节性红斑中的诊断意义

摘　　要：目的探讨T-淋巴细胞酶联免疫斑点法检测在结核性结节性红斑中的诊断意义。方法 2014年1月—2016年12月选取临床中的结节性红斑患者,行PPD皮试阳性(>10 mm),进行T淋巴细胞酶联免疫斑点法(T-SPOT.TB)检测,根据T-SPOT.TB的检验结果,进行分组治疗,以及是否对抗结核治疗有效,共得到4组数据。分别选取T-SPOT.TB在不同阳性值时,来对4组数据进行统计分析,应用敏感性、特异性、阳性预测值和阴性预测值描述T-SPOT.TB检测在诊断结核性结节性红斑中的作用价值。结果当设定T-SPOT.TB>10为阳性时,T-SPOT.TB的灵敏度、特异度、阳性、阴性预测值分别为92.50%、41.18%、78.72%、70.00%;当设定T-SPOT.TB>20为阳性时,分别为82.50%、70.59%、86.84%、63.16%。结论在临床上对疑似结核引起的结节性红斑患者行T-SPOT.TB检测对鉴别诊断是有帮助的,TSPOT.TB的阳性值越高,越支持结核感染。Objective This paper tries to study the value of detection of T-lymphocyte enzyme-linked immunospot assay in tuberculous nodular erythema. Methods Patients with erythema nodosum from January 2014 to Dece mber 2016 were selected and treatment with positive PPD skin test（〉10 mm）, and（T-SPOT.TB） was detected, and according to the Results of T-SPOT.TB, the patients were grouped for treatment as well as whether it is effective, a total of four sets of data were obtained. T-SPOT.TB was used to analyze the four groups of data at different positive values, and the sensitivity, specificity,positive predictive value and negative predictive value were used to describe T-SPOT.TB in the diagnosis of tuberculous nodular. Results When T-SPOT. TB 〉10 was positive, the sensitivity, specificity, positive and negative predictive value were 92.50%, 41.18%, 78.72%, 70.00%. When the T-SPOT. TB value〉20 was positive, the sensitivity, specificity, positive and negative predictive value were 82.50%, 70.59%, 86.84%, 63.16%. Conclusion T-SPOT. TB detection is helpful for diagnosis the trythema nodosum caused by tuberculosis. The higher of the positive value of T-SPOT. TB detection, the higher the possibility of tuberculosis infection.
