单颌和双颌拔牙矫治安氏Ⅱ类1分类错牙合的疗效比较

摘　　要：目的对单颌和双颌拔牙矫治安氏Ⅱ类1分类错牙合的疗效进行比较。方法方便选取该院2010年2月—2017年3月收治的50例安氏Ⅱ类1分类错牙合患者,按照患者矫治意愿分为观察组与参考组各25例,其中观察组接受上颌单牙合拔牙矫治方案,参考组接受双颌拔除4个前磨牙矫治方案,分别在治疗前、后观察两组患者X线头影测量结果。结果矫治前两组患者U1-FH、U1-AP、H角、UL-E、LL-E距离、Z角、SNA、SNB、ANB比较差异无统计学意义(P>0.05);治疗后两组患者U1-FH、U1-AP、H角、UL-E、LL-E距离、Z角指标均出现明显变化(P<0.05),两组患者治疗前、后SNA、SNB、ANB指标均未出现明显变化(P>0.05),观察组患者U1-FH、U1-AP、H角减小程度明显大于参考组(P<0.05),两组患者UL-E距离、H角、Z角治疗后比较差异无统计学意义(P>0.05);观察组矫治疗程为(22.13±4.53)个月,与参考组的(22.65±5.01)个月比较差异无统计学意义(P>0.05)。结论单颌与双颌拔牙矫治安氏Ⅱ类1分类错牙合能够使上前牙内收,使软组织侧貌很大程度改善,然而单颌拔牙矫治方案效果更佳,可在临床推广使用。Objective This paper tries to compare the effects of single and double jaw extraction treatment of class II Division 1 malocclusion. Methods 50 cases of class II division 1 malocclusion from Febuary 2010 to march 2017 were convenient selected and divided into the observation group and the reference group, with 25 cases in each group, according to the treatment methods. The observation group received single maxillary tooth extraction treatment scheme, the reference group received bimaxillary extraction of four premolars, cephalometric results before and after treatment of the two groups respectively were observed. Results U1-FH, U1-AP, UL-E, H angle, Z angle, LL-E distance, SNA, SNB, ANB of patients in the two groups before treatment had no significant difference（P〉0.05）; U1-FH, U1-AP, UL-E, H angle, LL-E distance, Z angle index were significantly changed of the two groups after treatment（P〈0.05）, SNA, SNB, ANB index were not changed significantly of patients in the two groups before treatment（P〉0.05）, U1-FH, U1-AP, H angle of the patients in the observation group were decreased significantly than the reference group（P〈0.05）, UL-E distance, H angle, Z angle after treatment of patients in the two groups had no significant difference（P〉0.05）; the treatment time for the observation group was（22.13±4.53）months, and the reference group was（22.65±5.01） months, and there was no significant difference（P〉0.05）. Conclusion The single and double jaw extraction treatment of class II division 1 malocclusion can make the anterior teeth retraction, the soft tissue profile can be greatly improved, but single maxillary extraction scheme of treatment effect is much better and can be applied in clinical practice.
