摘 要:目的:应用功能磁共振成像技术观察电针和手针对大脑功能的影响,探究电针和手针的中枢机制。方法:选择10名健康成年志愿者。根据"齐同对比"原则,即同一受试者在不同时间点分别接受不同的的刺激,包括电针针刺和手针针刺。两者采用随机的实验顺序且两者间隔至少1周,以减少针刺后效应对实验结果的影响。实验采用标准模块设计方案。电针组采用左侧上肢的外关和手三里,频率为2 Hz。手针组选取穴位同电针组,采用平补平泻,捻转速度每分钟60次。采用MATLAB软件下的SPM 12进行数据统计。结果:电针组激活的脑区,包括刺激穴位对侧颞中回、额中回、额下回、海马旁回,同侧的小脑后叶、枕中回、楔回,双侧中央前回和顶下小叶。手针组激活的脑区包括刺激穴位对侧的枕下叶、颞上回,同侧的颞上回、颞中回、顶下叶、额中回,双侧的中央后回。电针组和手针组存在共同激活的区域,电针组和手针组激活双侧的小脑,对侧的中央前回、中央后回、额中回、顶下叶、颞中回。电针组和手针组存在特异性激活的区域,电针组较手针组能更显著激活对侧的额中回、顶下小叶、小脑后叶,同侧的额上回。手针组较电针组能更显著激活对侧颞上回,同侧额上回、额中回、额内侧回、颞上回、颞中回、岛叶和顶下小叶。电针组激活的中央前回,不管是激活强度还是激活面积都较手组针显著。结论:电针组和手针组既存在共同激活的脑区,又存在特异性激活的脑区。电针组可能与大脑运动区域相关性更高。Objective: To observe the effect between electro-acupuncture (EA) and manual acupuncture using functional magnetic resonance image (fMRI), and to explore the intrinsic mechanism of different kinds of acupuncture. Methods: Ten healthy volunteers were enrolled in this study. According to the contrast "Uniform" principle, namely the same subjects at different time points to accept different acupuncture. Everybody was examined two different kinds of modalities of acupuncture, containing electro-acupuncture and manual acupuncture. And they were interval for at least one week. We used a typical block design. Acupuncture was performed at acupoint Waiguan (SJ-5) and Shousanli (LI-10) on the left upper. Current was delivered with a frequency of 2 Hz. The acupoint the manual acupuncture was as same as the group electro-acupuncture. The stimulation consisted of rotating the needle clockwise and counterclockwise for 1min at a rate 60 times per rain. And the patient felt "Deqi". The fMRI data was analyzed using SPM12. Results: Group EA activated brain regions primarily in the right Middle Temporal Gyrus, right Middle Frontal Gyrus, right Inferior Frontal Gyrus, fight Parahippocampa Gyrus, left Cerebellum Posterior Lobe, left Middle Occipital Gyrus, left Precuneus, bilateral Precentral Gyrus and bilateral Sub-lobar. Group MA activated brain regions primarily in the right Inferior Occipital Gyrus, fight Cerebellum Posterior Lobe,left Superior Temporal Gyrus, left Middle Temporal Gyrus, left Inferior Parietal Lobule, left Middle Frontal Gyrus, bilateral Postcentral Gyrus. There were common cerebral activated areas and specific cerebral activated areas responding to EA and MA. Both EA and MA were activated bilateral Cerebellum, right Precentral Gyrus, right Postcentral Gyrus, right Middle Frontal Gyrus, right Sub-lobar, right Middle Temporal Gyrus.
