胸腔镜手术治疗自发性气胸的临床疗效及安全性观察

摘　　要：目的评价胸腔镜手术治疗自发性气胸的临床疗效和安全性。方法选取我院收治的60例自发性气胸患者为研究对象,随机将其分为观察组和对照组,每组30例。观察组患者给予胸腔镜手术治疗,对照组患者给予常规手术治疗,观察并比较两组患者的临床疗效、手术时间、拔管时间、住院时间、术中出血量及术后并发症发生情况。结果治疗后,观察组患者的总有效率为90.00%,显著高于对照组的66.67%,差异有统计学意义(P<0.05)。观察组患者的手术时间、拔管时间、住院时间及术中出血量均显著优于对照组患者,差异有统计学意义(P<0.01)。两组患者的漏气、气胸复发等并发症发生率比较,差异均无统计学意义(P>0.05)。结论胸腔镜手术治疗自发性气胸的临床疗效优于常规手术疗法,建议临床推广。Objective To evaluate the clinical effect and safety of thoracoscopic surgery for treating spontaneous pneu-mothorax. Methods Sixty cases of spontaneous pneumothorax patients admitted in our hospital were enrolled as the study objects, and randomly divided into observation group and control group, with 30 cases in each group. The patients in the observation group were given thoracoscopic surgery, while the patients in the control group received open chest surgery. The clinical effect, operation time, extubation time, hospital stays, blood loss during the surgery, and complications after surgery were observed and compared between the two groups. Results After treatment, the total effective rate of the observation group was 90.00%, which was significantly higher than 66.67% of the control gruop, the difference was statistically significant （P〈0.05）. The operation time, extubation time, hospital stays and blood loss during the surgery in the observation group were significantly better than those in the control group （P〈0.01）. There was no significant differences in the incidence rate of leakage and pneumothorax recurrence and other complications between the two groups （P〉0.05）. Conclusion Thoracoscopic surgery has superior effect to open chest surgery in the treatment of spontaneous pneumothorax, which should be applied in clinic.
