摘 要:目的阐明安徽省艾滋病监测人群的HCV基因型分布特征,为分析HCV传播和流行特征提供科学依据。方法对安徽省艾滋病重点监测人群采血并进行HCV抗体初筛,阳性者使用巢式RT-PCR法对NS5B区和C/E2区进行扩增提取核酸,利用核酸测序、序列拼接和构建系统进化树对HCV基因进行分型。采用检验或确切概率法对重点监测人群来源、性别、年龄及感染途径进行基因型分布比较。结果不同来源的监测人群平均年龄为(35.4±9.46)岁,年龄差异不具有统计学意义(F=1.012,P>0.05);吸毒人群中HCV基因型主要为3b(39.47%)和3a型(21.71%),且出现三种混合型1b/3b、2a/3b和3a/3b型;暗娼人群中主要为1b(56.52%)和6a型(21.74%);男男性行为人群中主要基因型为1b型、2a和6a亚型;一般人群基因型主要为1b和2a型;不同感染途径的HCV患者其HCV基因型分布有所不同,差异有统计学意义(F=98.046,P<0.01)。结论安徽省重点艾滋病监测人群中HCV患者的HCV基因型共有8种亚型及3种混合型,基因型1a、3型和6型的首次发现提示了HVC病毒基因型的不断重组或变异,不同感染途径人群HCV基因型分布差异表明HCV病毒亚型在不同高危人群间有聚集特征。Objectlve We identified the hepatitis C virus (HCV) genotypes circulating within the main monitoring population of the human immunodeficiency virus (HIV) sentinel surveillance system in Anhui Province, and provided the scientific basis for the analysis of HCV transmission and epidemiology. Methods HCV NS5B region and C / E2 region were amplified the nucleic acid by nested real - time polymerase chain reaction (RT - PCR). The amplified products were sequenced and evaluated for gene alignment and typing. The comparison of the rates between different focus groups, sex, age, region and infection pathways were based on the E2 test or the exact probability method. Results The most frequent genotype in drug users (DES) was 3b (39.47%) ,followed by 3a (21.71%) and three mixed genotypes. The main genotypes in female sex workers (FSW) were lb (56.52%) and 6a (21.74%). Three HCV genotypes lb,2a,and 6a were detected in men who have sex with men (MSM) , and subtypes mostly lb and 2a, were detected in general populations. There was no significant difference in the distribution of HCV genotype among the drug users and the general population ( F = 9. 589 ,P 〉 0.05 ). The mean age was 35.4 ± 9.46 years, and single factor analysis of variance found that the differences in genotype distribution were not statistically significant (F = 1. 012, P 〉 1. 012 ). Differences in the distribution of HCV genotypes with different routes of infection were statistically significant (F = 98. 046, P 〈 0.01 ; Fisher's exact probability test). Conclusion We found eight single HCV subtypes in Anhui Province, including 1a, 1b ,2a,3a,3b ,6a,6n ,and 6u and three mixed genotypes ( 1b/3b ,2a/3b and 3a/3b). Different populations because of infection routes had significantly different genotypes distribution, and there was no significant difference in HCV genotype between age and sex. In addition, genotype 1 a,3 and 6 were first reported in this province.
