摘　　要：目的探讨漯河市妇女近3年HPV亚型感染特征及趋势。方法回顾性分析2014年1月-2016年8月我院妇产科门诊参加宫颈HPV分型检测的18 994例女性患者,分析感染情况及3年来感染趋势。结果 18 994例患者中,HPV感染率为22.5%,前5感染亚型分别为HPV16(5.3%),HPV52(3.5%),HPV58(2.9%),HPV53(2.6%),HPV39(2.1%);不同年龄组别中,15-25岁组(32.9%)和56-60岁组(29.7%)感染率较高;单纯高危亚型感染率为83.1%,高于单纯低危亚型感染(9.2%)及高+低危亚型感染(7.7%);一重感染率占74.6%,高于多重感染(25.4%)。2014、2015、2016年HPV总感染率分别为25.1%(1 531/6 093)、22.1%(1 794/8 128)、19.7%(944/4773),呈下降趋势。结论漯河地区妇产科门诊就诊患者中,15-25岁组和56-60岁组患者HPV感染率高,且以一重感染和高危感染为主,感染亚型以16、52、58较为常见;近三年漯河地区妇产科门诊HPV感染有下降趋势。Objective To explore the distribution and trend of HPV infection in opportunistic screening in Luohe city for nearly three years. Methods Data of 18 994 female patients who underwent HPV genotyping test in outpatient service of obstetrics and gynaecology department from January 2014 to August 2016 were retrospectively analyzed. Distribution and trend of HPV infection were analyzed for nearly three years. Results Among 18994 samples, the total infection rate of HPV was 22.5%. HPV16（5. 3% ）, HPV52 （3.5%）, HPV58 （2.9%）, HPV53 （2.6%） and HPV39 （2. 1% ） were the top five infection subtypes. The infection rates of 15 -25 （32.9%） and 56 -60 （29.7%） age groups were higher than other groups. In patients with HPV infection, the high - risk subtype infection group was 83.1%, higher than the pure low - risk subtype infection group （9.2%） and the high - risk ＋ low - risk subtype infection group （7.7%）. Meanwhile,a heavy infection rate accounted for 74. 6% ,higher than the multiple infections （25.4%）. The HPV infection rates in 2014,2015,2016 were 25.1% （1531/6093） , 22.1% （ 1794/8128 ）, 19.7 % （ 944/4773 ） respectively, which appeared a downward trend. Conclusion In the outpatients of gynaecology and obstetrics department in Luohe, 15 -25 years old and 56 -60 years old patients are prone to HPV infection. In patients with HPV infection, a heavy infection and high -risk infection accounted for the majority and HPV16,52,58 were more common in the past three years. The HPV infection had a downward trend.
