摘 要:目的:观察肾衰合剂对家兔急性肾盂高灌注压损伤的保护作用,研究其作用机制。方法:将30只新西兰家兔随机分为对照组、模型组及中药组,每组各10只,模型组及中药组家兔行双侧输尿管逆行插管后高压灌注,中药组给予肾衰合剂灌胃,观察各组家兔血清肌酐(SCr)、尿素氮(BUN)、内皮素(ET)、肿瘤坏死因子(TNF-α)及一氧化氮(NO)水平,测定肾小管上皮细胞钙离子浓度,并于光镜及电镜下观察肾组织变化。结果:肾衰合剂能显著降低肾盂高压灌注时家兔血SCr、BUN、ET、TNF-α水平,提高血NO水平,降低上皮细胞钙离子浓度,减轻肾小管上皮细胞破坏。结论:肾衰合剂通过改善肾血流、减轻炎性反应、拮抗钙离子释放,达到减轻肾盂高灌注压损伤的作用。Objective : To investigate the mechanism and protective effect of Shenshuai mixture on acute nephron injury induced by acute high - pressure perfusion in rabbit renal pelvis. Methods : Thirty New Zealand rabbits were divided into three groups randomly, including control group, model grooup and experimental group (n = 10 per group). The high- pressure perfusion models were built in both of the model group and experimental group through the high - pressure perfusion after bilateral ureteral stent implantation in rabbits. The experimental group was then treated with Shenshuai mixture through a gavage needle. The levels of serum creatinine (SCr) , urea nitrogen(BUN) , Endothelin (ET) , tumor necrosis factor ( TNF - α) and nitric oxide (NO) were calculated in those models. The concentration of calcium ion was examined in renal tubular epithelial cell. We also compared the potential change of pathology in kidney tissues through optical microscope and electron microscope. Results : The levels of SCr, BUN, ET and TNF - α were significantly downregulated, while NO was remarkably upregulated after the treatment of Shenshuai mixture. The concentration of calcium ion was also observed to be obviously reduced in experimental group, in which an injury of high - pressure perfusion had been weakened in renal tubular epithelial cell. Conclusion : Shenshuai mixture can impair the lesion induced by high - pressure perfusion of renal pelvis via improving renal blood flow, reducing the inflammatory reaction and resisting the release of calcium ion.
