摘　　要：目的分析心肌肌钙蛋白T(cTn-T)检测在充血性心力衰竭患者中的应用价值。方法将在我院接受治疗的2 000例充血性心力衰竭患者作为观察组,另抽选60例在我院接受健康体检的人员作为对照组,入院时间均在2014年1月至2016年5月。检测两组研究对象入院时的cTn-T、肌酸激酶(CK)以及肌酸激酶同工酶(CK-MB)水平。结果观察组患者的cTn-T、CK和CK-MB水平均显著高于对照组,且观察组中心功能Ⅳ级患者的cTn-T水平显著高于心功能Ⅱ级与Ⅲ级的患者,心功能Ⅲ级的患者的cTn-T水平显著高于心功能Ⅱ级的患者,差异均具有统计学意义(P<0.05),心功能Ⅱ级、Ⅲ级和Ⅳ级患者的CK、CK-MB水平比较,差异均无统计学意义(P>0.05)。观察组充血性心力衰竭患者中,死亡患者的cTn-T水平显著高于存活患者(P<0.05),死亡患者和存活患者的CK、CK-MB水平比较,差异无统计学意义(P>0.05)。结论 cTn-T与充血性心力衰竭患者的心功能密切相关,可作为充血性心力衰竭的早期诊断、患者的心功能判断以及近期预后判断的有效指标。Objective To explore the clinical value of cardiac troponin T （cTn -T） in patients with congestive heart failure. Methods Two thousand cases of patients with congestive heart failure in our hospital for treatment were selected as the study group, 60 cases who received physical examination in our hospital were selected as the control group. The time of admission was from January 2014 to May 2016. The levels of cTn-T, creatine kinase （CK） and creatine kinase isoenzyme （CK-MB） were detected in the two groups at admission. Results The levels of cTn-T, CK and CK-MB in the observation group were significantly higher than those in the control group, the level of cTn-T in patients with grade IV cardiac function was significantly higher than that in patients with grade II and III of cardiac function in the observation group, the level of cTn-T in patients with cardiac function III was significantly higher than that in patients with grade II cardiac function, the differences were statistically significant （P〈0.05）. There were no significant differences in the levels of CK and CK-MB in patients with cardiac function at grade II, III and IV（P〉0.05）. In the observation group, the cTn-T level of the dead patients were significantly higher than those of the survival patients （P〈0.05）, there were no statistically significant differences in the levels of CK and CK -MB between patients who died and those who survived （P〉0.05）. Conclusion cTn-T are closely related to cardiac function in patients with congestive heart failure, which can be used as an early indicator in the diagnosis of congestive heart failure, as well as an effective indicator of cardiac function and recent prognosis of patients.
