选择性断流术和联合断流术治疗门脉高压症的临床效果比较

摘　　要：目的探究选择性断流术和联合断流术治疗门脉高压症的临床效果。方法选取在我院接受治疗的门脉高压症患者60例为研究对象,入院时间均在2015年4月至2016年3月,将其随机分为对照组(n=30)和观察组(n=30),观察组患者接受选择性断流术治疗,对照组患者接受联合断流术治疗,对比两组患者手术后的门静脉压力下降情况(FPP)、手术时间、出血量、住院时间、术后近期消化道出血复发和食管胃底静脉曲张改善情况。结果两组患者手术后的的FPP、手术时间、出血量、住院时间及术后近期消化道出血复发率比较,差异均无统计学意义(P>0.05);手术后,观察组患者的食管胃底静脉曲张消失率为96.67%,与对照组的66.67%比较,差异具有统计学意义(P<0.05)。结论选择性断流术治疗门脉高压症效果显著,安全性高,可有效改善患者的食管胃底静脉曲张情况,值得在临床中进一步推广实施。Objective To compare the clinical effect of selective devascularization and combined devascularization on portal hypertension. Methods Sixty cases of patients with portal hypertension in our hospital from April 2015 to March 2016 were selected as the research objects, and were randomly divided into the control group （n=30） and the observation group （n=30）. The observation group received selective devascularlzation, and the control group underwent combined devascularization. The pressure of portal vein after treatment （FPP）, time of operation, amount of bleeding, duration of hospitalization, recent recurrence of gastrointestinal bleeding and improvement of gastroesophageal and gastric varices after treatment were compared between the two groups. Results There were no significant differences in FPP, operation time, bleeding volume, hospitalization time and the recurrence rate of the recent gastrointestinal bleeding between the two groups （P〉0.05）. After treatment, the disappearance rate of gastroesophageal and gastric varices in the observation group was 96.67%, compared with 66.67% in the control group, the difference was statistically significant （P〈0.05）. Conclusion Selective devascularization on portal hypertension is effective and safe, which can effectively improve the esophageal and gastric varices of patients. It is worthy of further promotion in clinical practice.
