急性脑卒中后睡眠障碍相关因素分析

摘　　要：【目的】了解急性脑卒中后睡眠障碍(post-stroke sleep disorder,PSSD)的发生状况和相关因素。【方法】采用匹兹堡睡眠质量指数(pittsburgh sleep quality index,PSQI)量表作为评判睡眠障碍的指标。采用SPSS统计学软件对两组资料进行处理,等级资料采用RIDIT分析,计数资料采用卡方检验,计算其优势比(OR)值和95%的可信区间等以说明相关因素与急性脑卒中后睡眠障碍的关系。【结果】急性脑卒中后睡眠障碍(PSQI总分>7分)的发生率为18.66%。睡眠障碍患者在年龄、糖尿病、并发症(肺炎、泌尿系感染)、经济水平、病变的部位、精神状态、脑卒中的性质、美国国立卫生研究院卒中量表(NIHSS)评分等各方面分布与非睡眠障碍患者有差异(P<0.05)。【结论】急性脑卒中后睡眠障碍发生率高,卒中的部位、年龄、性质、并发症及NIHSS评分水平、心理状态等因素可能与急性脑卒中后睡眠障碍有关。【Objective】To investigate the incidence and related factors of sleep disorder（pssd） in patients with acute stroke.【Methods】Using the Pittsburgh sleep quality index（PSQI） scale as an indicator of sleep disorders.Using SPSS statistical software to process the data of the two groups,the hierarchical data was analyzed by RIDIT,and the counting data was tested by chi-square,which calculated its advantage ratio（OR） value and 95% confidence interval,To illustrate the relationship between related factors and sleep disorder after acute stroke.【Results】The incidence of post-stroke sleep disorders（PSQI score7 points） was 18.66%.The patients with Sleep disorders were different in age,diabetes,and（Pneumonia,urinary tract infection）,economic level,location of the lesion,mental state,the nature of stroke,the National Institutes of Health Stroke Scale（NIHSS） score and other aspects（P〈0.05）.【Conclusion】the high incidence of acute sleep disorder after stroke,and the location,age,nature of the stroke,complications and NIHSS score level,the in the mind state factors,such as positive factors intervention,supplemented by drug treatment prognosis is good.
