尺骨冠状突前内侧骨折的手术治疗

摘　　要：目的探讨尺骨冠状突前内侧骨折的诊断及治疗方法。方法自2011年6月至2013年12月共收治10例肘尺骨冠状突前内侧骨折患者。男7例，女3例；年龄19—52岁，平均33．8岁。O’Driscoll冠状突骨折分型均为Ⅱ型，其中1亚型5例，2亚型4例，3亚型1例。所有患者均采用微型钢板内固定冠状突和锚钉修复外侧副韧带。结果10例患者术后获得12—28个月（平均15．8个月）的随访。平均屈曲121°（90°-140°），平均伸直丢失18°（0°～50°），平均旋前70°（50°-90°），平均旋后69°（51°-90°）。根据MEPS肘关节功能评分：优5例，良4例，可1例。结论正确诊断冠状突前内侧骨折，早期微型钢板支撑内固定和锚钉修复损伤外侧副韧带，可获得满意的临床疗效。Objective To investigate the diagnosis and treatment of anteromedial facet fracture of coronoid process of ulna. Methods From June 2011 to December 2013, 10 cases of anteromedial facet fracture ofcoronoid process of ulna were treated. There were 7 males and 3 females, with age ranging from 19 to 52 years （mean, 33.8 years）. All the fractures were type 11 according to O＇Driscoll classification, including 5 cases of subtype-1, 4 eases of subtype-2 and 1 case of subtype-3. Anteromedial facet fracture of coronoid process of ulna was fixed by mini plate and lateral collateral ligament was repaired by anchor in all the patients. Results All the patients were follow-up for 12 to 28 months with an average of 15.8 months. The average flexion was 121° （range, 90° to 140°）, the average extension loss was 18° （range, 0 to 50°）, the average pronation was 70° （range, 50° to 90°）, and the average supination was 69° （range, 51° to 90°）. According to MEPS elbow performance score, the results were excellent in 5 cases, good in 4 cases and fair in 1 case. Conclusion Accurate diagnosis of anteriomedial facet coronoid fracture, early mini plate fixation and anchor repair of the lateral collateral ligament can achieve satisfactory clinical results.
