摘  要:目的研究有限元分析应用于胸腰椎压缩性骨折治疗过程中的结构力学特性。方法选取T11~L2椎体压缩性骨折患者10例,根据其210层CT影像学检查结果建立T11~L2椎体压缩性骨折的有限元模型,对已完成的模型实施有效验证后模拟并建立过伸手法复位三维模型,根据分析计算结果探究受压缩椎骨复位前后形状变化以及各位置压力分布情况。结果通过构建胸腰椎压缩性骨折的三维有限元模型,其模拟外观直观清晰且具有结构力学应力分析能力,给予手法复位后其压缩的椎体出现较为明显的过伸,同时,在其过程应用的手法复位原理与经典的胸腰椎体力学试验结果一致。三维有限元分析相比传统研究方法更为精确可靠,其成本较低,更易于操作研究。结论利用建立三维有限元分析技术研究胸腰椎压缩性骨折以及手法复位的力学特点,其结果直观、精确,易于分析比对且操作简易,值得广泛应用及推广。Objective To analyze the treatment mechanisms of thoracolumbar spine compression fracture by hyperextension based on 3D finite element method. Methods Totally 10 cases with compression fracture of T11~ L2 were selected into our study. Based on the 210-layer consecutive CT DICOM3. 0 format images,a 3D finite element model of hyperextensive manipulative reduction for compression fracture of T11~ L2 was built after the effect of model was confirmed. The analysis of the result of the shape and the stress distribution of the model was carried out based on the results. Results Through the 3D finite element model construction of thoracic and lumbar vertebral compression fracture,the simulated appearance was clear and intuitive,which had mechanical structure stress analysis ability. After given manipulative reduction,the compressed vertebra developed hyperextension. At the same time,the results in the manipulative reduction principle and the classic mechanical test were the same,which meant that 3D finite element method was durable,precise and laconic. Conclusion The 3D finite element analysis is more accurate and reliable than the traditional research methods with easy analysis and simple operation,which is worthy of further application and promotion.
