摘　　要：目的研究螺钉大小、长度及方向参数,指导经皮钉-棒内支架治疗骨盆前环骨折。方法采集2014年3月—2015年6月在郴州市第一人民医院行骨盆计算机断层扫描的正常骨盆CT资料,男性45例,女性45例;年龄28~68岁,平均41岁。将CT数据导入Mimics10.0软件,测量螺钉长度、直径、方向参数。结果男、女髂骨内螺钉长度分别平均为(144.25±5.56)、(140.57±5.97)mm,螺钉直径分别平均为(13.26±1.35)、(12.16±1.00)mm,螺钉与人体冠状面的夹角分别为(51.07±4.32)°、(48.88±4.40)°,螺钉与人体矢状面的夹角分别为(24.71±2.54)°、(23.64±2.20)°;钉道角度左右对比差异无统计学意义(P>0.05);男女对比差异有统计学意义(P<0.05)。螺钉骨性通道长度左右对比差异无统计学意义(P>0.05),男女对比差异有统计学意义(P<0.05);螺钉直径左右对比差异无统计学意义(P>0.05),男女对比差异有统计学意义(P<0.05)。结论在把握好螺钉大小、长度及方向的情况下,在前下棘与髋臼上缘之间到髂后上棘之间置入螺钉是比较安全的,值得在骨科临床中推广。Objective To study the diameter,length and direction of screw for guiding the operation of percutaneous pedicle srew internal fixation device for anterior pelvic fractures. Methods Normal pelvic computed tomography scans of adult patients from Mar. 2014 to Jun. 2015 obtained in Chenzhou No. 1 People＇s Hospital were collected. There were 45 males and 45 females,with age of 28-68（ average,41） years. CT images were imported into Mimics 10. 0 for reconstruction,and the parameter of screw＇s length,diameter and direction were measured. Results The length of screw in ilium was（ 144. 25 ± 5. 56） mm in male and（ 140. 57 ± 5. 97） mm in female. The diameter of screw was（ 13. 26 ± 1. 35） and（ 12. 16 ± 1. 00） mm in male and female,respectively. The angles of screw and human coronal was（ 51. 07 ± 4. 32） ° in male and（ 48. 88 ± 4. 40） ° in female. The angle of screw and human sagittal was（ 24. 71 ± 2. 54） ° in male and（ 23. 64 ± 2. 20） ° in female. According to the screw＇s angle contrast analysis of left and right,there was no statistical difference（ P〉0. 05）. For the screw＇s angle contrast analysis of man and female,the difference was of statistical significance（ P〈0. 05）. Compared with left,the length of right screw had no statistical difference（ P〉0. 05）. Compared with female,the length of male had difference of statistical significance（ P〈0. 05）. According to the diameter of screw contrast analysis of left and right,there was no statistical difference（ P〉0. 05）. Compared with the female,the diameter of male had difference of statistical significance（ P〈0. 05）. Conclusion In case of assuring right diameter,length and direction in operation,inserting screw in bony channel between anterior superior iliac spine and acetabulum roof to the posterior superior iliac spine is feasible and safe. It is worth promoting in clinic.
