针刺阳明经穴结合补阳还五汤口服治疗外伤性周围神经断裂伤的疗效研究

摘　　要：目的探讨采用针刺阳明经穴结合补阳还五汤治疗外伤性周围神经断裂伤的临床疗效。方法纳入2014年1月至2016年12月期间我院收治的外伤性周围神经断裂伤患者共80例,随机分为两组各40例。对照组采用腺苷钴胺注射液进行肌内注射治疗,观察组采用针刺阳明经穴结合补阳还五汤口服治疗。比较两组患者的基本功能、神经电生理情况。结果经Ridit分析基本功能结果表明,对照组R=0.619,95%CI=(0.537,0.701);观察组R=0.381,95%CI=(0.314,0.448);两组无可重复区间,差异具有统计学意义(P<0.05)。经Ridit分析神经电生理结果表明,对照组R=0.627,95%CI=(0.545,0.709);观察组R=0.373,95%CI=(0.304,0.442);两组无可重复区间,差异具有统计学意义(P<0.05)。结论针对外伤性周围神经断裂伤患者采用针刺阳明经穴联合补阳还五汤治疗能有效改善患者的神经及运动功能,提高临床疗效。Objective To explore the clinical effect of acupuncture at Yangming meridian combined with oral administration of Buyang Huanwu decoction in treating traumatic peripheral nerve fracture-damage. Methods A total of 80 cases of patients with traumatic peripheral nerve fracture-damage admitted to our hospital from January 2014 to December 2016 were selected and randomly divided into two groups, with 40 cases in each group. The control group was given cobamamide injection for intramuscular injection, while the observation group was given acupuncture at Yangrning meridian combined with oral administration of Buyang Huanwu decoction. The basic function and electroneurophysiology were compared between two groups. Results The Ridit analysis of basic function showed that the R value and 95% CI value were 0.619 and （0.537, 0.701） in Control group, and 0.381 and （0.314, 0.448） in observation group; No repeatable interval was found, and the difference between two groups was statistically significant （P〈0.05）. The Ridit analysis of electroneurophysiology showed that the R value and 95%CI value were 0.627 and （0.545, 0.709） in control group, and 0.373 and （0.304, 0.442） in observation group; No repeatable interval was found, and the difference between two groups was statistically significant （P〈0.05）. Conclusions Acupuncture at Yangming meridian combined with Buyang Huanwu decoction in the treatment of traumatic peripheral nerve fracture-damage can effectively improve patients＇ neurological and motor fimction, and improve the clinical effect.
