1期联合手术治疗外伤性鼻骨骨折鼻中隔偏曲的临床观察

摘　　要：目的探讨鼻内镜下联合手术治疗外伤性鼻骨骨折鼻中隔偏曲的疗效及安全性。方法选择外伤性鼻骨骨折伴鼻中隔骨折偏曲患者70例,随机分为两组各35例。对照组行单纯鼻骨骨折整复术,实验组行1期鼻骨骨折整复联合鼻中隔矫正术。结果术后,实验组的鼻腔外形及鼻腔通气VAS评分均显著低于对照组,鼻腔容积、鼻总呼气量与鼻腔最小横截面积均显著大于对照组(P<0.05);随访期间,实验组的优良率显著高于对照组,并发症发生率显著低于对照组(P<0.05)。结论对于外伤性鼻骨骨折伴鼻中隔偏曲患者,实施1期鼻骨复位联合鼻中隔偏曲矫正术对改善患者的鼻通气功能及减少塌鼻有重要意义。Objective To explore the efficacy and safety of endoscopic combined operation for the treatment of traumatic nasal bone fracture complicated with nasal septum deviation. Methods 70 patients with traumatic nasal bone fracture complicated with nasal septum deviation were randomly divided into two groups equally. The control group received diaplasis of nasal bone fracture, while the experimental group received one-stage diaplasis of nasal bone fracture and septoplasty. Results After operation, the VAS scores of nasal cavity shape and nasal ventilation of experimental group were lower than those of control group, and the volume of nasal cavity, the total expiratory volume of nose and the minimum cross sectional area of nasal cavity were higher than those of control group （P 〈0.05）. During follow-up period, the excellent and good rate of experimental group was higher than that of control group, and the incidence of complications was lower than that of control group （P〈0.05）. Conclusions One-stage diaplasis of nasal bone fracture and septoplasty for patients with traumatic nasal bone fracture complicated with nasal septum deviation has important significance in improving the nasal ventilation function and reducing flattened nose.
