摘  要:目的分析不同糖化血红蛋白(Hb Alc)水平的糖尿病患者的血液流变学特点及并发症情况。方法根据糖化血红蛋白水平将糖尿病患者分为三组:A组201例:Hb Alc 6.5%~7.0%;B组176例:Hb Alc 7.01%~8.5%;C组166例:Hb Alc>8.5%。另选取100例健康人群为D组。比较四组的血液流变学指标,并比较A组、B组和C组患者的并发症发生率。结果糖尿病患者(A组、B组、C组)的全血粘度(低切、中切、高切)、血浆粘度以及血沉方程K值均显著高于健康人群(D组),P<0.05。糖尿病患者的并发症发生率随着Hb Alc的升高而升高,差异有统计学意义(P<0.05)。相关性分析显示,糖尿病患者的各项血液流变学指标均与Hb Alc呈正相关关系(P<0.05)。结论糖尿病患者的血糖控制水平与血液流变学的改变程度及并发症发生率密切相关,应密切关注血液流变学指标的改变,及早预防和控制糖尿病慢性并发症的发生发展。Objective To analyze the characteristics of hemodynamics and complications of diabetic patients with different levels of glycated hemoglobin (HbAlc). Methods According to patients' HbAlc levels, diabetic patients were divided into group A (n = 201, HbAlc 6.5% - 7.0%), group B (n = 176, HbAlc 7.01% - 8.5%) and group C (n = 166, HbAlc 〉 8.5%). Another 100 cases of healthy population were selected as group D. The indicators of hemodynamics of four groups were compared, and the incidences of complications of group A, group B and group C were compared. Results The whole blood viscosity (low cut, medium cut and high cut), plasma viscosity and K value of ESR equation of diabetic patients (group A, group B and group C) were significantly higher than those of healthy population (group D), P〈0.05. The incidence of complications of diabetic patients increased along with the increase of HbAlc level (inter-group comparison, P 〈0.05). Correlation analysis showed that the indicators of hemodynamics of diabetic patients were positively related with HbAlc (P 〈0.05). Conclusions The glycaemic control level of diabetic patients is closely related with the change of hemodynamics and incidence of complications, which suggests that we should pay attention to the change of hemodynamics indicators closely, so as to prevent and control the occurrence and development of chronic complications of diabetes mellitus.
