PPH术结合外痔切除一期缝合术治疗环状混合痔50例的临床分析

摘　　要：目的探讨PPH结合外痔切除一期缝合术在环状混合痔治疗中的临床疗效。方法将2015年6月~2016年6月入住我院肛肠外科的100例环状混合痔患者招募入组。随机将患者分为研究组(n=50)和对照组(n=50)。研究组行PPH结合外痔切除一期缝合术治疗,对照组行外剥内扎术,观察两组术后疗效、并发症及恢复情况。结果研究组治愈率显著高于对照组(94%vs 74%,P<0.05),术后并发症(出血、水肿、延迟拆线)显著少于对照组,且住院天数短于对照组[(6.7±2.3)d vs(7.8±3.4)d,P<0.05]。结论 PPH结合外痔切除一期缝合术治疗环状混合痔具有疗效满意、术后并发症少、缩短住院天数等优势,是治疗环状混合痔的理想改良术式。Objective To investigate the clinical effect of PPH combined with external hemorrhoidectomy primary suture in the treatment of annular mixed hemorrhoids. Methods A total of 100 patients with annular mixed hemorrhoids admitted in the department of anorectal surgery of our hospital from June 2015 to June 2016 were enrolled. These patients were randomly divided into study group(n=50) and control group(n=50). The study group was treated with PPH combined with external hemorrhoidectomy primary suture, and the control group was treated with external stripping and internal ligation surgery. The curative effect after surgery, complication and recovery were observed. Results The cure rate of the study group was significantly higher than that of the control group(94% vs 74%, P<0.05). The postoperative complications(bleeding, edema and delayed stitches) in the study group were significantly less than those in the control group, and the hospital stay in the study group was shorter than that of the control group[(6.7 ±2.3)d vs(7.8±3.4)d,P <0.05]. Conclusion PPH combined with external hemorrhoidectomy and primary suture for the treatment of annular mixed hemorrhoids has advantages of satisfactory curative effect, less postoperative complications and shorter hospital days. It is an ideal modified procedure for the treatment of circular mixed hemorrhoids.
