摘 要:目的通过对糖尿病肾病患者的视黄醇结合蛋白(RBP4)和尿微量白蛋白(mAlb)含量的测定并分析相关性,评估RBP4在糖尿病肾病患者的疾病诊断及治疗预后中的作用。方法选择2015年8月—2016年7月期间广西柳州市工人医院肾内科收治的2型糖尿病合并糖尿病肾病的80例患者为研究组,同时随机抽取80名健康体检者为对照A组,随机抽取80例2型糖尿病无肾病患者为对照B组。测定受试者的肝功能、肾功能相关指标,包括血清肌酐(Cr)、血清尿素氮(BUN),测定尿mAlb、尿RBP4含量,并分析尿RBP4与BUN、mAlb等的相关性。结果对照A组受试者血清Cr、血清BUN、尿mAlb和尿RBP4等含量,显著低于研究组和对照B组患者,差异有统计学意义(P<0.05)。研究组患者血清Cr、血清BUN、尿mAlb和尿RBP4等含量,显著高于对照B组患者,差异有统计学意义(P<0.05)。RBP4与BUN和mAlb均呈正相关(P<0.05)。结论 RBP4与UREA、mAlb呈正相关,可作为早期糖尿病肾病患者肾脏功能损伤的指标。Objective Through the measurement and correlative analysis of retinol binding protein 4(RBP4) and urinary microalbumin(mAlb) content in diabetic nephropathy patients, the role of RBP4 in diagnosis and treatment of diabetic nephropathy patients is to be evaluated. Methods 80 patients with type 2 diabetes mellitus and diabetic nephropathy treated in nephrology department in Liuzhou Worker's Hospital from August 2015 to July 2016 were randomly selected as the study group, and 80 healthy physical examiners were randomly selected as the control group A, and 80 patients with type 2 diabetes and without nephropathy were selected as the control group B. Liver function, renal function and other related indicators, including serum creatinine(Cr), serum urea nitrogen(BUN), urinary mAlb and urinary RBP4 content were determined,and the correlation between urinary RBP4 and BUN, mAlb etc. were analyzed. Results Serum Cr, BUN, urine mAlb and urine RBP4 content in the control group A was significantly lower than the study group and the control group B. The differences were statistically significant(P〈0.05). The levels of serum Cr, serum BUN, urinary mAlb and urinary RBP4 content in study group was significantly higher than that of control group B, and the differences were statistically significant(P〈0.05).RBP4 was positively correlated to BUN and mAlb(P〈0.05). Conclusion RBP4 is positively related to UREA and mAlb, and it can be used as an indicator of renal function impairment in early diabetic nephropathy patients.
