髂静脉闭塞继发股青肿的手术和腔内治疗

摘　　要：目的:探讨静脉切开取栓术和血管腔内技术治疗髂静脉闭塞继发股青肿的临床疗效。方法:2013年8月至2016年12月,收治髂静脉闭塞继发股青肿患者共6例,5例采用静脉切开取栓术、同期髂静脉闭塞球囊扩张、支架植入术;1例采用导管溶栓术、残余髂静脉闭塞行球囊扩张及支架植入术。结果:所有患者均取得技术成功,共植入支架8枚,其中1例植入3枚。无手术并发症,术后随访无复发、深静脉血栓后遗症状轻微,复查血管支架通畅。结论:静脉切开取栓术同期内支架术治疗髂静脉闭塞继发PCD疗效确切、并发症少,导管溶栓术、残余髂静脉腔内治疗也可取得良好疗效,但建议适用于进展较慢的股青肿患者。Objective： To investigate the clinical effect of surgical thrombectomy and endovascular treatment in phlegmasia eerulea dolens caused by iliac vein occlusion. Mothods： A total of six patients with phlegmasia cerulea dolens caused by iliac vein occlusion who were admitted to our hospital from August 2013 to December 2016 were enrolled. Among these patients, five underwent surgical thrombectomy, as well as balloon dilation and stent implantation for lilac vein occlusion, and one underwent catheter-directed thrombolysis, as well as balloon dilation and stent implantation for residual iliac vein occlusion. Results： All patients achieved a technical success. A total of 8 stents were implanted, and one patient underwent the implantation of three stents. There were no postoperative complications or recurrence during follow-up, there were slight sequela of deep venous thrombosis, and reexamination showed stent patency. Conclusions： Venous thrombectomy combined with simultaneous stent implantation has a marked clinical effect and few complications in the treatment of phlegmasia cerulea dolens caused by iliac vein occlusion. Catheter-directed thrombolysis combined with endovascular treatment of residual iliac vein occlusion can also achieve a good clinical effect, but this technique is recommended for patients with slowly progressive phlegmasia cerulea dolens.
