摘 要:目的研究骨髓间充质干细胞(BMSCs)子宫局部移植修复宫腔粘连(IUA)子宫内膜损伤的效果。方法本研究模型为IUA新西兰大白兔,使用刮宫和感染双重损伤法建立模型,为实验组(模型组和移植组),将行假手术的新西兰大白兔作为对照组。移植组在建模1周后采用PKH26标记的BMSCs进行局部移植到双侧子宫。成功移植后4周,观察并记录子宫内膜形态变化;计算所有模型的腺体计数和纤维化面积比;采用免疫组化(IHC)对子宫内膜腺上皮的CK和VIM蛋白表达进行检测;采用荧光显微镜定位BMSCs。结果实验组在移植4周后的子宫内膜腺体计数较对照组相比差异有统计学意义(P_(模型)=0.00,P_(移植)=0.01),模型组与移植组比较差异有统计学意义(P=0.00)。对照组的子宫内膜纤维化面积明显大于模型组(P=0.00),而较移植组差异无统计学意义(P=0.53)。各组CK蛋白表达水平比较差异无统计学意义(P=0.17)。VIM蛋白表达情况,模型组与对照组比较差异有统计学意义(P_(模型)=0.00),对照组与移植组比较差异无统计学意义(P=0.92)。移植组在移植4周后的宫颈、阴道及卵巢组织中均可观察到少量红色荧光,其余两组均未发现红色荧光存在。结论 BMSCs子宫局部移植可使IUA新西兰大白兔获得较正常子宫内膜相似的子宫内膜形态学和细胞表型,对临床BMSCs移植治疗IUA有价值。Objective To study the effect of bone marrow mesenchymal stem cells (BMSCs) transplantation in uterus on repair of endometrial injuries of intrauterine adhesion (IUA) . Methods New Zealand white rabbits with IUA were selected as the study model, which was established by double trauma method with curettage and infection, those rabbits were regarded as experimental group (model group and transplantation group), and the rabbits undergoing sham operation were selected as control group. The rabbits in transplantation group were locally transplanted with PKH26-labeled BMSCs into bilateral uteruses after 1 week of modeling. At four weeks after a successful transplantation, the morphological changes of endometrium were observed and recorded, and the count of glands and the area ratio of fibrosis in all the rabbit models were calculated. Immunohistochemistry (IHC) was used to detect the expressions of CK and VIM protein in epithelium of endometrium, and BMSCs were localized by fluorescence microscopy. Results There was statistically significant difference in the number of endometrial glands at 4 weeks after transplantation between experimental group and control group (Pmodel = 0. 00, Ptransplantation = 0. 01 ) . There was statistically significant difference between model group and transplantation group (P= 0. 00) . The area of endometrial fibrosis in control group was statistically significantly larger than that in model group (P = 0. 00), but compared with transplantation group, there was no statistically significant difference (P = 0. 53) . There was no statistically significant difference in the expression level of CK protein among different groups (P = 0. 17) . There was statistically significant difference in the expression level of VIM protein between model group and control group (Pmodel =0. 00), but the difference between control group and transplantation group was not statistically significant (P = 0. 92) .
