12例妊娠哺乳期乳腺癌诊疗体会

摘　　要：目的:探讨妊娠哺乳期乳腺癌的诊断与治疗。方法:回顾性分析我院2005年1月—2010年1月收治的12例妊娠哺乳期乳腺癌病例资料,总结诊断、治疗经验及教训。结果:术后病理9例浸润性导管癌,1例浸润性小叶癌,1例为湿疹样乳腺癌,1例为炎性乳腺癌。Ⅰ期1例(8.3%);Ⅱ期6例(41.7%);Ⅲ期4例(29.7%);Ⅳ期1例(8.3%)。1例于治疗后1年多死亡,5年生存7例(58.3%),其中1例对侧乳房转移,2例骨转移、1例出现对侧乳房转移及远处气管转移,余患者均未出现复发及转移。结论:妊娠哺乳期乳腺癌,因乳房生理性增大导致乳腺肿块不易触及,应重视产前、哺乳期的乳腺检查。早期干预意义重大。Objective： To investigate the diagnosis and treatment of breast cancer during pregnancy or lactation. Methods： Data from 12 cases of patients with breast cancer admitted in our hospital from January, 2005 to January, 2010 were retrospectively analyzed. The experience and lessons for their diagnosis and, treatment were summarized. Results： There were 9 cases of invasive ductal carcinoma, 1 case of invasive lobular carcinoma, lcase of eczema breast cancer, 1 case of inflammatory breast cancer, in which 1 case was in stage I （8.3%）, 6 cases in stage Ⅱ （41.7%）, 4 cases in stage Ⅲ （29.7%） and 1 case in stage IV （8.3%）. One case survived for more than 1 year and 7 cases for 5 years （58.3%）after treatment. There were 1 case of contralateral breast metastasis, 2 cases of bone metastasis, and 1 case of contralateral breast metastasis and distant tracheal metastasis, and recurrence and metastasis were not found in the other patients. Conclusion： Breast cancer occurred during pregnancy or lactation is difficult to be found by touch diagnosis due to the physiological enlargement of breast and great attention should be paid to prenatal and lactating breast check. Early intervention is of great significance.
