摘　　要：目的 探讨应用上臂外侧穿支皮瓣游离移植修复手部皮肤缺损的临床疗效.方法 2012年1月-2016年1月收治手部皮肤软组织缺损患者16例,皮肤缺损面积3.0 cm×5.5 cm^4.0 cm×7.0 cm,采用上臂外侧穿支皮瓣Ⅰ期游离移植修复,皮瓣切取面积3.5 cm×6.5 cm^4.3 cm×8.5 cm.术后定期随访,总结分析皮瓣成活情况及修复手部的外观与功能情况.结果 1例皮瓣远端部分坏死,经换药2周后愈合,其余皮瓣成活良好.术后随访6~12个月,皮瓣外观及弹性良好,手功能和外形恢复满意,皮瓣供区瘢痕轻,无明显功能障碍.结论 游离上臂外侧穿支皮瓣移植修复手部皮肤软组织缺损临床效果满意,是修复手部中、小面积皮肤软组织缺损的一种有效方法.Objective To explore the clinical effect of free lateral arm perforator flap on the repair of hand skin defect. Methods From January 2012 to January 2016, our department treated 16 cases with hand skin and soft tissue defects, the skin defect areas ranged from 3.0 cm × 5.5 cm^4.0 cm × 7.0 cm. Using free lateral arm perforator flap to repair skin defects, the flaps were cut into an area of 3.5 cm × 6.5 cm^4.3 cm × 8.5 cm. The survival status of the flap and the appearance and function of the hand were analyzed. Results 1 case of the distal part of the flap had necrosis, given wound dressing and healed after 2 weeks. The rest of the flaps survived well. Postoperative follow-up ranged from 6 to 12 months, the flaps appearance and elasticity were good, hand function and shape recovery were improved, the wounds of flap donor areas were little without obvious dysfunction. Conclusion The clinical effect of free lateral arm perforator flap transplantation to repair the skin and soft tissue defects is satisfactory. It is an effective method to repair the hands with small areas of skin and soft tissue defects.
