摘　　要：目的加味大黄承气合生长抑素治疗对重症胰腺炎患者炎症因子、肝肾功能及临床疗效的影响。方法选取2013年1月-2016年2月本院收治的82例重症胰腺炎患者为研究对象,依据患者入院顺序均分为对照组及联合组各41例,2组均给予常规对症支持治疗,对照组在此基础上加上生长抑素进行治疗,联合组在对照组基础上联合加味大黄承气汤进行治疗,观察治疗后2组临床疗效、炎症因子、肝肾功能指标、预后状况及并发症发生率。结果治疗后,联合组总有效率明显高于对照组(P<0.05);治疗后,2组IL-6、IL-8、TNF-α、ALT、AST、SCr、BUN、APACHEⅡ评分较治疗前明显降低,且治疗后联合组各项指标变化程度较对照组明显(P<0.05)。结论加味大黄承气汤联合生长抑素治疗对重症胰腺炎患者临床疗效显著,有效降低患者炎症因子表达及并发症发生率,并保护肝肾脏器功能,且预后效果佳。Objective To enhance the effect of Supplemented rhubarb purgative decoction and somatostatin on inflammatory factors,liver and kidney function and clinical efficacy in patients with severe acute pancreatitis.Methods From January 2013 to February 2016,82 patients with severe pancreatitis were enrolled in this study.All patients were divided equally into control group and combined group according to their admission sequence.Both groups were given routine symptomatic treatment.Besides,the control group was treated with somatostatin,and the combined group was treated with the Supplemented rhubarb purgative decoction on the basis of the control group.The clinical effects of the two groups were observed.The levels of interleukin-6（IL-6）,Interleukin-8（IL-8）,tumor necrosis factor-α（TNF-α）],liver and kidney function [alanine aminotransferase（ALT）,aspartate aminotransferase（AST） Creatinine（SCr）,blood urea nitrogen（BUN）,prognosis [acute physiology and chronic health scoring system Ⅱ（APACHE Ⅱ） score] and the incidence of complications.Results The total effective rate of the combined group was significantly higher than the control group（P〈0.05）.After treatment,the scores of IL-6,IL-8,TNF-α,ALT,AST,SCr,BUN and APACHE Ⅱ were significantly lower than those before control（P 0.05）.Conclusion Supplemented rhubarb purgative decoction combined with somatostatin treatment has significant clinical effect on patients with severe acute pancreatitis,effectively reducing the expression of inflammatory factors and the incidence of complications.It can protect the liver and kidney function,and the prognosis is good.
