摘 要:目的了解深圳市某综合医院14年住院死亡病例的疾病分布和变化趋势,为降低该地区死亡率及指导医院科学发展提供参考依据。方法采用Excel 2007和SAS 9.1软件对2003-2016年的3 975例死亡病例进行数据整理和回顾性分析。结果 14年来,该院住院患者病死率呈现逐渐下降的趋势,平均病死率为0.72%,男性患者病死率高于女性,每年的12月、1月、2月期间住院患者病死率较高。20岁以前的住院患者,年龄段在15~19岁的病死率较高(为0.41%),<1岁的病死率也较高(为0.25%);在20岁以后,随着年龄的增长病死率呈逐渐上升趋势。威胁人群健康的前3位死因是肿瘤、循环系统疾病和呼吸系统疾病,占77.21%;重症医学科、肿瘤科、微创介入科、心内科、消化内科、神经外科、呼吸内科和血液内科为所有临床科室死亡病例的主要构成科室,占83.1%。结论医院可针对不同性别、年龄、月份、病种、科室等方面对医疗资源进行调配,开展多样化、多层次的医疗服务工作,从而提高住院患者的存活率和生存质量。Objective To study the distribution and trend of death cases during 14 years in a general.hospital in Shenz- hen, and to provide reference for reducing the mortality rate and guiding the scientific development of the hospital. Methods The data of 3 975 cases of death from 2003 to 2016 were analyzed retrospectively with Excel 2007 and SAS 9.1 software. Results In the past 14 years, the mortality rate of inpatients in the hospital showed a decreasing trend, with an average mortality rate of 0. 72%. The fatality rate of male patients was higher than that of female. The mortality rate of inpatients in December, January and February was higher each year. Among inpatients below 20 years old, the mortality rate of inpatients from 15 years old to 19 years old was the highest (0.41%), followed by those below one year (0.25%). Inpatients over 20 years old showed a gradual increase in mortality rate with the growth of age. The first three causes of death were cancer, circulatory system diseases, and re- spiratory diseases, accounting for 77.21%. Departments including Intensive Care, Oncology, Minimally Invasive Intervention, Cardiology, Gastroenterology, Neurosurgery, Respiratory Medicine and Hematology were the main departments of death in all clinical departments, accounting for 83.1%. Conclusion The hospital can allocate medical resources in different sex, age, month, disease, department and so on to carry out diversified and multi-level medical service work, so as to improve the survival rate and quality of life of inpatients.
