摘 要:目的 介绍一种加药用侧孔针引导早产儿无导丝式PICC腔内心电图定位的方法.方法 选择2016年3—6月行PICC早产儿30例,将加药用侧孔针应用于早产儿无导丝PICC置管腔内心电图实时定位,置管后通过X线定位判断定位效果.结果 加药用侧孔针引导早产儿无导丝式PICC腔内心电图定位特异性为86.7%(26/30),置管后X线胸片定位,一次性置管成功率为83.3%(25/30).结论 加药用侧孔针引导实现了早产儿无导丝式PICC腔内心电图实时定位,通过ECGⅡ导联P波变化可以判断导管尖端位置,取材方便,操作简便,效果可靠,值得推广.Objective To introduce a method in which side-holed needles for dosing were used to guide endocardial electrogram localization on peripherally inserted central catheter (PICC) in premature infants. MethodsTotally 30 premature infants who received PICC between March and June 2016 were selected. The side-holed needles for dosing were used to guide endocardial electrogram localization on PICC in premature infants. And localization effects were measured after catheterization.Results The specificity of these side-holed needles for dosing used for endocardial electrogram localization on PICC in premature infants was 86.7% (26/30). After catheterization,X-ray localization were conducted on their chest,with a success rate of catheterization once was 83.3% (25/30).Conclusions The side-holed needles for dosing help to achieve real-time endocardial electrogram localization on PICC in premature infants. With ECG Ⅱ connected to P waves,the tip of catheter can be localized,and they are easily accessible,operable and reliable,thus worthy of clinical expansion.
