摘　　要：目的探讨临床药师在业务大查房中对合理用药的促进作用。方法对76份临床药师参与业务大查房后填写的药学点评汇总表(包含科室药品管理、医嘱用药、不良反应上报情况)及整改情况进行总结分析。结果 76份药学点评汇总表(共456份病历)中,有181份病历存在不合理情况,占39.69%;不合理总例次为209,整改总例次为177,整改率为84.69%。结论临床药师在业务大查房中将不合理用药情况及时反馈给科室,督促科室进行整改,对合理用药具有很好地促进作用。Objective To promote the role of clinical pharmacists in rational drug use in the grand round. Methods Totally 76 pharmacy reviews （containing department of drug administration, medical orders and ADR case reports） written by clinical pharmacists after the grand round and rectification situations were summarized and analyzed. Results Among the 76 pharmacy reviews with 456 medical records, 181 （39.69%） were reasonable, 209 were unreasonable, and 177 of them （84.69%） were rectified. Conclusion Clinical pharmacists provide timely feedback to unreasonable drug use to the departments after the grand round and promote rectification and clinical rational drug use.
