摘 要:目的 探讨后踝解剖型接骨板治疗踝关节周围骨折的临床疗效与应用价值.方法 2012年8月-2015年2月选择诊治的踝关节周围骨折患者84例,根据入院顺序分为治疗组与对照组各42例,治疗组采用后踝解剖型接骨板内固定治疗,对照组采用传统钢板内固定治疗,观察两组预后恢复情况.结果 治疗组手术时间、术后住院时间与骨折愈合时间分别为(41.34±10.32)min,(6.982.14)d与(11.40±1.49)周,均明显低于对照组的(56.33±9.28)min,(8.24±1.84)d与(14.87±2.94)周(P〈0.05).治疗组与对照组术后6个月的踝关节功能评分为(89.144.24)分和(81.45±4.89)分,均明显高于术前的(64.25±5.98)分和(64.19±6.14)分(P〈0.05),且治疗组术后6个月的踝关节功能评分明显高于对照组(P〈0.05).治疗组术后6个月的内固定松动、软组织感染、畸形愈合、骨髓炎、切口感染等并发症发生情况明显少于对照组(P〈0.05).结论 后踝解剖型接骨板治疗踝关节周围骨折具有更好的微创性,能促进骨折愈合,减少并发症的发生,从而改善踝关节功能,具有很好的临床应用价值.Objective To investigate the application effects of ankle periarticular fractures treated by the ankle bone anatomical plate. Methods From August 2012 to February 2015 in our hospital, selected 84 ankle periarticular fractures for treatment and were equally divided into treatment group and control group of 42 patients under a hospital order, the treatment group received ankle bone anatomical plate treatment, the control group were given the traditional plate fixation, the recovery prognosis were observed. Results The operative time, postoperative hospital stay and fracture healing time in the treatment group were (41.34 ± 10.32) min,(6.98±2.14) d and(11.40±1.49) weeks respectively that were significantly lower than the control group of(56.33±9.28) min, (8.24±1.84) d and(14.87±2.94) weeks(P〈0.05). The postoperative 6 moths ankle function scores in the treatment group and control group were (89.14±4.24)points and (81.45±4.89) points, that were significantly higher than the preoperative of (64.25±5.98) points and (64.19±6.14) points (P〈0.05), while the treatment group were significantly higher (P〈0.05). The postoperative 6 moths loosening, soft tissue infections, abnormal healing, osteomyelitis, wound infection and other complications in the treatment group were significantly less than the control group (P〈0.05). Conclusion The application of ankle bone anatomical plate in the treatment of ankle periarticular fractures has better minimally invasive that can promote healing and reduce complications, thereby improve ankle function that has good clinical research values.
