Intertan髓内钉在股骨转子周围骨折中的应用

摘　　要：目的 探讨Intertan髓内钉髓内固定在治疗股骨转子周围骨折（Peritrochanteric femoral fracture,PFF）的临床疗效.方法 收集2011年7月-2015年12月诊治的PFF患者128例,根据治疗方法的不同分为观察组与对照组各64例,对照组采用经皮加压钢板内固定治疗,观察组采用Intertan髓内钉内固定治疗.记录两组手术时间、术中出血情况、住院天数、术后并发症,采用Harris评分标准评估髋关节功能恢复情况.结果 观察组的住院时间、术中出血量、手术时间均明显少于对照组（P〈0.05）.术后6个月进行评定,观察组髋关节功能Harris评分优良率为92.2%,对照组为81.3%,观察组明显高于对照组（P〈0.05）.观察组术后6个月的切口感染、 静脉血栓形成、 髋内翻、 股骨干骨折等并发症发生率为7.8%,对照组为23.4%,观察组均明显少于对照组（P〈0.05）.结论 采用Intertan髓内钉内固定治疗PFF时,具有创伤小、愈合快、髋关节功能恢复效果快等优点,值得临床推广.Objective To investigate the clinical efficacy of Intertan intramedullary nail in the treatment of Peritrochanteric femoral fracture （PFF）. Methods From July 2011 to December 2015, 128 PFF patients with the diagnosis and treatment in our hospital were selected, all the patients were equally divided into the observation group and control group according to the different treatment methods. The control group was treated with percutaneous compression plate internal fixation, the observation group was treated with intramedullary nail internal fixation, recorded in patients with operation time, intraoperative bleeding, postoperative complications, hospital stay, Used the Harris hip score to evaluate the hip function recovery. Results The operation time, blood loss and length of stay in the observation group were significantly less than those in the control group （P〈0.05）. After 6 months, the excellent and good rate of Harris score in the observation group was 92.2%, the control group was 81.3%, and the observation group was significantly higher than that in the control group （P〈0.05）. After 6 months, the incision infection, venous thrombosis, hip inversion, femoral shaft fractures and other complications incidence rate in the observation group was 7.8%, while the control group was 23.4%, the observation group was significantly less than the control group （P〈0.05）. Conclusion Internal intramedullary nail in the treatment of PFF has the featuresof the small wound, the fast healing, the quick hip function recovery effect, the satisfactory clinical effect, it is worth to be popularized.
