摘  要:目的探讨“龙虾钳”样复拇指畸形的手术方法。方法2012年10月-2016年1月,对5例“龙虾钳”样复拇指畸形采用切除副指,咬除第1掌骨远端膨大的部分关节面,主指近节指骨截骨矫形,指问关节侧副韧带紧缩(1例指间关节融合),拇短展肌止点重建的方法进行手术治疗。结果5例术后随访6-24个月.矫形拇指外形改善,拇指对指及外展功能正常,感觉正常,MP关节屈曲30°-50°、背伸0°~15°,IP关节屈曲0°~15°、背伸0°~10°,术后随访2例出现指间关节侧方偏斜,由于年龄较小,继续随访。结论切除副指。主指近节指骨截骨矫形.拇短展肌止点重建对“龙虾钳”样复拇指畸形矫形是一种可行的手术方法。Objective To investigate the operation method of the lobster pliers-hke thumb duplication. Methods From October 2012 to January 2016, surgical treatment was implemented for five patients of lobster phers-like thumb duplication by removing the deputy finger, biting away articular surface of the distal portion of the first metacarpal enlargement, proximal phalanx osteotomy of the main finger was conducted, tightening collateral ligament of interphalangeal joint (1 patient of interphalangeal joint fusion), reconstructing abductor pollicis brevis TDC. Results 5 cases were followed up for 6 -24 months after the surgery, the appearance of the orthopedic thumb was improved, opposition and abduction of thumb, feeling was normal, MP joint flexion 30°-50°, dorsiflexion 0°-15°, IP flexion 0°-15°, dorsiflexion 0°-10°, 2 patients had the interphalangeal joint lateral deflection, during follow up due to younger age of patients, the follow-up would be continued. Conclusion Removal of the deputy finger, proximal phalanx osteotomy of the main finger is conducted, reconstructing abductor pollicis brevis TDC is a viable surgical method for the lobster pliers-hke thumb duplication.
