摘  要:目的探讨血清抑制性G蛋白α亚基-2(Giα-2)和中性粒细胞明胶酶相关脂质运载蛋白(NGAL)水平在2型糖尿病合并不同分级高血压患者中的改变及临床意义。方法选取197例2型糖尿病患者,分为单纯2型糖尿病(DS组)45例、2型糖尿病合并高血压1级(DH1组)47例、2型糖尿病合并高血压2级(DH2组)51例、2型糖尿病合并高血压3级(DH3组)54例;另选取40例查体无糖尿病和高血压者作为对照组。应用酶联免疫吸附法测定各组血清Giα-2和NGAL水平。多元线性回归法分析Giα-2和NGAL的影响因素。结果 DS组Giα-2水平较对照组降低,DH1、DH2、DH3组Giα-2水平均高于DS组,其中DH3组最高(P均<0.05);DS组NGAL水平较对照组升高,在DH1、DH2、DH3组中,随着高血压分级增加,NGAL水平逐渐增加(P均<0.05)。多元线性回归分析结果显示:Giα-2与收缩压呈正相关(r=0.54,P<0.05),NGAL水平与收缩压、空腹血糖呈正相关(r=0.55、0.14,P均<0.05)。进一步多元回归分析提示收缩压是血清Giα-2水平的独立影响因素,收缩压、空腹血糖是血清NGAL水平的独立影响因素。结论 2型糖尿病合并高血压患者血清Giα-2和NGAL水平均升高,Giα-2和NGAL水平变化对2型糖尿病合并高血压的发生均有一定影响。Objective To investigate the changes of serum inhibitory G-protein subunit Gi α-2( Giα-2) and neutrophil gelatinase-associated lipocalin( NGAL) in type 2 diabetes combined with different classifications of hypertension and its clinical value. Methods Totally 197 patients with type 2 diabetes were selected. They were divided into 45 cases of simple diabetes( DS group),47 cases of type 2 diabetes combined with grade 1 hypertension( DH1 group),51 cases of type 2 diabetes and grade 2 hypertension( DH2 group),and 54 cases of type 2 diabetes and grade 3 hypertension( DH3group). Meanwhile,40 people without diabetes and hypertension were enrolled as the control group. The serum Giα-2 and NGAL levels in each group were detected by ELISA. The influence factors of Giα-2 and NGAL were analyzed by multiple linear regression analysis. Results The level of Giα-2 in the DS group was lower than that in the control group,at the same time,the levels of Giα-2 in the DH1 and DH2 and DH3 groups were higher than that of the DS group,and the DH3 group was the highest( all P〈0. 05). The level of NGAL in the DS group was higher than that of the control group,with the increase of hypertension grades,NGAL increased in the DH1 and DH2 and DH3 groups( all P〈0. 05). The results of the multivariate linear regression analysis showed that Giα-2 was positively correlated with systolic blood pressure( SBP)( r= 0. 54,P〈0. 05),and NGAL was positively correlated with SBP and fasting blood-glucose( FBG)( r = 0. 55 and 0. 14,both P〈0. 05). Further multiple linear regression analysis showed that SBP was an independent influence factor for Giα-2level,meantime,SBP and FBG were independent influence factors for NGAL level. Conclusion Serum levels of Giα-2and NGAL increase in patients with type 2 diabetes and hypertension,and the changes in levels of Giα-2 and NGAL havea certain influence on the occurrence of type 2 diabetes and hypertension.
