摘　　要：目的探讨JCI标准下运用医疗失效模式与效应分析方法 (Healthcare Failure Mode and Effect Analysis,FMEA)评估产科患者跌倒薄弱环节,探究跌倒风险的发生原因,并提出有针对性的改进措施,从而有效降低产科患者跌倒风险事件的发生率。方法在JCI标准下,比较FMEA运用于预防患者跌倒前后产科患者的跌倒发生率,采用FMEA质量改进方法,成立分析小组、绘制流程图、列出潜在失效模式,查找及分析关键失效模式相关的根本原因,提出改善措施并落实。结果实施FMEA管理办法后,患者跌倒失效模式中5项高风险失效模式的RPN值明显低于实施前(P<0.05);住院患者跌倒例次低于实施前。结论 JCI标准下应用FMEA管理办法,有效减少了产科患者跌倒护理风险事件的发生,确保患者安全,值得推广。Objective To explore the application of healthcare failure mode and effect analysis（FMEA） under JCI standard in evaluating weak parts of preventing falls among obstetric patients, to understand where the risks of falls come from, and to put forward improvement approaches, so as to reduce the incidence of falls among obstetric patients effectively. Method Compared the incidence of falls before and after FMEA was applied under JCI standard, employed FMEA quality improvement method, set up analysis group, drew flow chart, listed potential failure modes, found out and analyzed the fundamental causes for key failure modes, put forward improvement approaches and implemented. Results After the application of FMEA, the RPN values of five high risk failure modes were significantly lower than those before the application of FMEA（P〈0.05）, and the cases of falls among obstetric patients declined. Conclusion The application of FMEA management measures under JCI standard reduced the cases of risk events of falls among obstetric patients effectively and therefore ensured higher safety, so it was worthy of being generalized.
