大剂量阿片类药物控制患者疼痛的护理管理与体会

摘　　要：目的总结150例使用大剂量及超大剂量阿片类药物患者爆发痛护理管理的经验。方法对150例使用大剂量及超大剂量阿片类药物患者爆发痛处理的管理经验进行总结;护理重点是做好阿片类药物全程安全管理,运用LOS镇静评分早期发现及处理嗜睡及过度镇静。正确使用吗啡静脉注射处理爆发痛,提高患者生活质量。结果 150例重度癌痛患者中CR50例,PR75例,MR25例,NR0例,CR+PR=83.3%,住院两周和出院时患者生活质量评分均高于住院前,对出现的不良反应进行及时有效处理。结论对使用大剂量及超大剂量阿片类药物患者做好爆发痛管理可以提高重度癌痛患者生活质量,有效保证患者的用药安全。Objective To summarize the nursing experience of 150 cases of patients with breakthrough pain taking high and super-high dose of opioids. Method Nursing experience of 150 cases of patients with breakthrough pain taking high doses of opioids was summarized; full-access security management of opioids was an important part of nursing; LOS sedation scores was employed for early detecting and addressing of sleepiness and excessive sedation. Proper morphine intravenous injection was used to control breakthrough pain, in order to improve the quality of life of patients. Results Among the 150 cases of patients with severe cancer pain, there were 50 cases of CR, 75 cases of PR, 25 cases of MR, 0 case of NR, CR＋PR=83.3%. At 2 weeks after admission and at discharge, score of quality of life was higher than that before admission. Adverse reactions were timely and effectively treated. Conclusion Good management of breakthrough pain for patients taking high and super-high dose of opioids could improve the quality of life of patients, and ensure the safety of medication.
