摘  要:目的探讨自身免疫甲状腺疾病(AITD)与25(OH)D水平的相关性。方法选取初诊Graves病(GD)42例,缓解期GD 38例,慢性淋巴细胞性甲状腺炎(HT)24例,健康对照组38例。检查血清25(OH)D、促甲状腺受体抗体(TR-Ab)、抗甲状腺过氧化物酶抗体(TPO-Ab)、总甲状腺素(TT_4)、总三碘甲状腺原氨酸(TT_3)、游离甲状腺素(FT_4)、游离三碘甲状腺原氨酸(FT_3)、促甲状腺素(TSH)水平。并对25(OH)D水平、甲状腺功能及甲状腺抗体进行相关性分析。结果与对照组相比,初诊GD组、缓解GD组及HT组的血清25(OH)D均显著下降,差异有统计学意义(P<0.05)。GD组(初诊期、缓解期)血清25(OH)D与FT_3、FT_4、TT_3、TT_4TSH TR-Ab水平无相关性。HT组25(OH)D与TPO-Ab、FT_3、FT_4、TT_3、TT_4之间无相关性(P>0.05)。结论 AITD患者血中25(OH)D水平的不足,显示其可能在甲状腺自身免疫反应的发生、发展中起重要作用。Objective To investigate the correlation between autoimmune thyroid disease (AITD) and the level of 25 - hydroxy vita- min D3. Methods Forty two cases of newly diagnosed Graves disease (GD) , 38 cases of GD in remission, 24 cases of Hashimoto's thy- roiditis (HT) and 38 cases of healthy control group were selected. Serum 25 - hydroxyvitamin D3 ,thyrotropin receptor antibody (TRAb) , anti thyroid peroxidase antibody (TPOAb) , total thyroxine (TT4 ) , total three triiodothyronine ( TT3 ) , free thyroxine ( FT4 ) , free three triiodothyronine ( FT3 ), thyrotropin (TSH) were measured were tested. Analysis of the correlation between 25 - hydroxyvitamin D3 levels and thyroid function and thyroid antibody was performed. Results Compared with the control group, the serum levels of 25 - hydroxyvita- min D3 in newly diagnosed GD group and GD group in remission and HT group were significantly decreased, and the difference was statis- tically significant. Serum 25 -hydroxyvitamin D3 levels were not associated with the levels of serum TT3, TT4, FT3, FT4, TSH ,TRAb in GD group (initial diagnosis and remission). There was no correlation between 25 - hydroxyvitamin D3 and TPOAb, TT3 , TT4 , FT3 , FT4 and TSH in HT group(P 〉 0.05). Conclusion The level of 25 -hydroxyvitamin D3 in AITD patients is insufficient, which shows that it may play an important role in the occurrence and development of thyroid autoimmune reaction.
