清热化瘀方药浴治疗寻常性银屑病(血热证)临床观察

摘　　要：目的探讨清热化瘀方药浴治疗寻常性银屑病(血热证)临床疗效。方法将170例符合纳入标准的寻常性银屑病(血热证)患者按照随机数字表法分为2组,每组85例,两组均给予消银胶囊口服,同时治疗组每晚采用清热化瘀方药浴治疗,对照组每晚采用婴儿沐浴液洗浴,疗程均为4周,比较两组患者治疗前、后中医证候积分、银屑病皮损面积和严重程度指数(PASI)评分、皮肤病生活质量指数(DLQI)评分、血清白细胞介素-17a(IL-17a)水平,并比较两组患者临床疗效。结果治疗组和对照组分别有81例和79例纳入研究,疗程结束后,两组主要中医证候皮疹灼热感、瘙痒和心烦易怒积分、PASI评分、DLQI评分及血清IL-17a水平均较治疗前下降(均P<0.01),且治疗结束后治疗组主要中医证候积分低于对照组(P<0.01),PASI评分、DLQI评分及血清IL-17a水平均低于对照组(P<0.05),临床疗效优于对照组(P<0.05),差异均有统计学意义。结论清热化瘀方药浴可有效改善寻常性银屑病(血热证)型患者皮损状况和中医证候,提高生活质量,疗效显著。Objective To investigate the clinical curative effect of Qingre Huayu bath on PsoriasisVulgaris（Blood Heat Type ）Methods One hundred and seven patients with psoriasis vulgaris（blood heat syndrome）were divided into two groups according to the digital random table method,with eighty cases in each group. Both groups were given Xiaoyin capsule orally. The treatment group were treated with Qingrehuayu medicated bath every night,while the control group used shower bath with baby bath cream every night,a course of 4 weeks. TCM syndrome integral,psoriasis skin lesion area and severity index（PASI）score,dermatology life quality index（DLQI）score,serum interleukin-17 a（IL-17 a）level before and after treatment of the patients of the two groups were compared,and the clinical curative effect of the two groups were also compared.Results Eighty-one cases of the treatment group and 79 cases of the control group were included into the studies. The main syndromes of tetter burning sensation,itching,and distractedness score,PASI score,DLQI score and the level of serum IL-17a were lower than they were before the treatment（all P〈0.01）. The main TCM syndromes of the treatment group was lower than that of the control group（P〈0.01）; PASI score,DLQI score and the level of serum IL -17a were lower than those of the control group（all P〈0.05）; The clinical curative effect was better than that of the control group（P〈0.05）. The difference was statistically significant.Conclusion Qingre Huayu medicated bath can effectively improve the skin condition and TCM syndrome of（blood heat type）life,so it has significant effect.
