摘 要:目的探讨3D打印技术在Lisfranc骨折脱位的临床应用。方法采集6例Lisfranc骨折脱位患者足踝部CT断层扫描数据,进行三维重建和3D打印制作,在模型进行模拟手术,并将其应用于临床诊治、手术规划及手术参照。结果所有患者全部获得随访,随访时间12~14.5个月,平均12.5个月,均在3D打印实体模型指导下顺利完成手术,术后无皮肤坏死、感染、克氏针松动及足弓塌陷。6例患者术中所见与3D打印实体模型骨折脱位结果变化一致。结论采用数字化技术重建跖跗关节骨折脱位模型,3D打印技术1:1打印骨折脱位模型,能直观反应Lisfranc损伤立体形态及分型,对临床医生的诊治、手术计划等有较大帮助,减少手术时间及手术出血量,达到较好的手术效果。Objective To explore the clinical application of 3D printing in Lisfranc fracture dislocation. Methods Lisfranc CT scanning data of 6 cases with ankle fracture dislocation were collected, and 3-D reconstruction and 3-D printing was conducted. The model was simulated and applied to clinical diagnosis, surgical planning and surgical operation. Results All the patients were followed up for 12-14.5 months with an average of 12.5 months. All the patients received a successful operation under the guidance of 3-D solid model. There was no skin necrosis, postoperative infection, loosening of Kirschner wire or arch collapse. 6 cases were consistent with the results of 3-D printing solid model in terms of the change in fracture dislocation. Conclusion Using digital technology to reconstruct tarsometatarsal joint dislocation model and using 3-D printing for the 1:1 printing model can directly reflect the damage of Lisfranc fracture, three-dimensional form and type, which can be of great help for the diagnosis, treatment and surgical planning. It can reduce bleeding quantity and operation time, and achieve better operation results.
