鼻内镜联合超声刀在儿童扁桃体切除术中的应用

摘　　要：目的探讨鼻内镜联合超声刀在儿童扁桃体切除术中的应用价值。方法 48例需行扁桃体切除术的患儿随机分为观察组与对照组,每组24例。观察组行鼻内镜联合超声刀扁桃体切除术,对照组行传统扁桃体剥离术。观察并比较两组术中出血量、手术时间、住院时间、术后白膜脱落时间、住院费用、疼痛程度及并发症情况。结果观察组术中出血量明显少于对照组(P<0.05),白膜脱落时间长于对照组(P<0.05),手术时间和住院时间明显短于对照组(P<0.05);术后6、24、48、72 h的疼痛评分均低于对照组(P<0.01)。不良反应与住院费用两组比较差异无统计学意义(P>0.05)。结论鼻内镜联合超声刀行儿童扁桃体切除术是一种更微创、更易操作的术式,能提高医疗质量,值得推广。Objective To explore the value of nasal endoscope combined with ultrasonic scalpel in children tonsillectomy. Methods 48 patients requiring tonsillectomy were divided into Observation Group and Control Group, 24 patients in each group. Observation Group received the tonsillectomy with nasal endoscope combined with ultrasonic scalpel while Control Group received traditional tonsillectomy. The intraoperative blood loss, operation time, hospital stay, time of postoperative albuginea shedding, hospitalization cost, pain degree and complications of the two groups were observed and compared. Results Observation Group had the intraoperative bleeding loss significantly less than Control Group （P〈0.05）, the time of postoperative albuginea shedding significantly longer than Control Group （P〈0.05）, the operation time and hospital stay significantly shorter than Control Group （P〈0.05）, and the pain scores at 6 h, 24 h, 48 h and 72 h postoperatively lower than Control Group （P〈0.01）. There was no statistical difference between the two groups in terms of adverse reactions and hospitalization cost （P〉0.05）. Conclusion The pediatric tonsillectomy with endoscopy combined with ultrasonic scalpel is a more minimally invasive and easier operation. It can improve the medical quality and worth clinical promotion.
