摘　　要：目的了解新生儿发生宫内窘迫时血乳酸以及心肌酶水平的变化。方法选取宫内窘迫新生儿50例(A组),同时选取窒息的新生儿(B组)和正常新生儿(C组)50例作为对照,检测其脐动脉血(出生时)及出生后3、7 d的血乳酸、肌酸激酶(CK)、肌酸激酶同工酶MB(CKMB)、乳酸脱氢酶(LDH)、α-羟丁脱酸氢酶(HBDH)水平,并对其相互关系进行分析。结果 A组新生儿出生时和出生后3 d的血乳酸、CK、CKMB、LDH和HBDH水平均显著高于C组,明显低于B组(P<0.01);出生后7 d时与对照组比较则差异无统计学意义(P>0.05)。宫内窘迫新生儿的脐带血乳酸水平与CK、CKMB均呈正相关性(P<0.05)。结论宫内窘迫可影响新生儿的血乳酸和心肌酶水平,脐动脉血乳酸检测可协助诊断和评估宫内窘迫。Objective To investigate the changes in the blood lactate and myocardial enzyme level in the newborns with intrauterine distress. Methods Fifty newborns with intrauterine distress （Group A） were selected while fifty newborns with asphyxia （Group B） and fifty normal newborns （Group C） were selected as control. The blood lactate, creatine kinase （CK）, creatine kinase isoenzyme MB （CKMB）, lactate dehydrogenase （LDH） and alpha-hydroxytryptophanase （HBDH） level of umbilical artery （at birth） and 3d, 7d after birth were detected, and their mutual relationship was analyzed. Results The levels of blood lactate, CK, CKMB, LDH and HBDH in Group A at birth and 3 d after birth were significantly higher than those in Group C, but lower than those of Group B （P〈0.01）; there was no significant difference between Group A and control groups at 7d after birth （P〉0.05）. The blood lactate and myocardial enzyme levels of umbilical artery of the newborns with intrauterine distress were positively correlated with CK and CKMB （P〈0.05）. Conclusion Intrauterine distress may affect the blood lactate and myocardial enzyme level of umbilical artery. Blood lactate detection can help to diagnose and evaluate intrauterine distress.
