胸膜外入路Nuss手术与Nuss手术的单中心临床疗效对比研究

摘　　要：目的在单中心情况下,对比胸膜外入路Nuss手术和Nuss手术的安全性和可行性。方法2015年1月~2016年12月45例行Nuss手术者,排除漏斗胸复发、有合并症同期手术、大于13岁、广泛凹陷使用两根钢板支撑和极重度和严重非对称性患者,余36例随机分两组,16例胸膜外入路Nuss手术组和20例Nuss手术组。比较两种不同术式围手术期患者情况、并发症及手术效果,并观察胸膜外入路组钢板是否明确在胸膜外。结果 36例均顺利完成手术,在两组年龄、Haller指数差异无统计学意义的前提下,手术优良率、手术时间、术中出血量和住院时间差异亦均无统计学意义。结论胸膜外入路Nuss手术安全可行,可作为同常规Nuss手术相同的手术选择。Objective To compare the safety and feasibility of the thoracoscopic extrapleural Nuss surgery and Nuss surgery in the case of single center. Methods Patients were collected between January 2015 and December 2016, 45 Nuss operations were taken place, despite of the recurrence of funnel chest and with some complications at the same time, the age was more than 13 years, and widely using two root steel support and very severe and serious asymmetry patients. There were 36 cases which randomly divided into two groups, 16 cases of thoraco- scopie extrapleural Nuss surgery and 20 cases of Nuss surgery. The conditions, complications and surgical effects of the two different groups of operative perioperative patients were com- pared, and whether the steel plates of the outer pleura were clearly outside the pleura was ob- served. Results All of the 36 cases, which had no statistical difference of age and Hailer in- dex, were under surgery successfully, and the rate of good operation, operation time, intraop- erative blood loss and the length of time differences had no statistical significance. Conclusion The thoraeoscopic extrapleural Nuss surgery is safe and feasible, which can be used as the same procedure as conventional Nuss surgery.
