摘 要:目的探讨超声弹性成像及超声造影对甲状腺结节的诊断价值。方法回顾性分析60个甲状腺结节弹性成像及超声造影表现,且均经手术或穿刺活检病理证实其良恶性。以病理结果作为金标准,分别计算常规超声、弹性成像、超声造影以及弹性成像联合超声造影诊断甲状腺结节的灵敏度、特异度和准确度。结果常规超声诊断甲状腺结节良恶性的灵敏度、特异度、准确度分别为60.7%、66.7%、63.3%,弹性成像诊断甲状腺结节良恶性的灵敏度、特异度、准确度分别为72.7%、74.1%、73.3%,超声造影诊断甲状腺结节良恶性的灵敏度、特异度、准确度分别为75.7%、81.5%、78.3%,弹性成像联合超声造影诊断甲状腺结节良恶性的灵敏度、特异度、准确度分别为81.8%、85.2%、83.3%。结论在常规超声检查的基础下,弹性成像与超声造影技术可提高甲状腺结节良恶性的诊断准确率,两者联合可提高甲状腺结节的鉴别诊断能力。Objective To compare the clinical value of elasticity imaging and contrast-en- hanced ultrasound (CEUS) in diagnosis of thyroid nodules. Methods Elasticity imaging and contrast-enhanced ultrasound manifestation of 60 thyroid nodules were retrospectively analyzed, and all nodules were confirmed by surgery or biopsy pathology of the benign or malignant. With postoperative pathology as golden standard, the sensitivity, specificity and accuracy for identif- ying the property of thyroid nodule were calculated using conventional ultrasound examination, elasticity imaging, CEUS and the elasticity imaging combined with CEUS. Results The sensi- tivity, specificity and accuracy using conventional ultrasound examination were 60.7%, 66.7% and 63.3%, respectively; 72.7%, 74. 1% and 73.3% using elasticity imaging, re- spectively; and 75.7% , 81.5% and 78.3% using CEUS, respectively. While the elasticity imaging combined with CEUS were 81.8% , 85.2% and 83.3% , respectively. Conclusion With conventional ultrasound examination as the foundation, elasticity imaging and CEUS could significantly improve the diagnostic accuracy. The elasticity imaging combined with CEUS could improve the ability of differential diagnosis of thyroid nodules.
