摘 要:目的探索通过雷帕霉素调控自噬对脊髓损伤大鼠脊髓神经元凋亡的作用及机制。 方法将36只SD大鼠随机分为3组,每组12只。假手术组仅行椎板切除,生理盐水组和雷帕霉素组采用改良Allen’s法损伤T10脊髓。术后1、3、7、14 d评价大鼠神经功能,术后14 d采用免疫组织化学双标染色检测神经元自噬水平,检测自噬微管相关蛋白1轻链3(LC3)和Beclin-1 mRNA和蛋白的表达。 结果1、3、7、14 d生理盐水组和雷帕霉素组术后斜板实验(28.14±2.48)°、(30.65±3.58)°、(34.85±4.12)°、(37.62±4.73)°;(29.02±2.64)°、(36.37±3.87)°、(43.28±4.39)°、(50.06±4.52)°(P=0.430、0.021、0.016、0.005)和改良Tarlov评分(0.23±0.12)、(0.96±0.22)、(1.17±0.28)、(1.73±0.35)分;(0.29±0.15)、(1.34±0.23)、(2.26±0.30)、(3.68±0.41)分(P=0.390、0.037、0.028、0.013),均较假手术组显著降低,之后有增加趋势,自3 d起两组差异有统计学意义。运动诱发电位(MEP)检测生理盐水组和雷帕霉素组潜伏期与波幅(17.08±0.39)、(13.85±0.31)、(9.26±0.23)、(8.24±0.25) ms;(1.86±1.05)、(2.37±1.26)、(3.84±1.75)、(4.27±1.49) μV和(12.63±0.34)、(9.88±0.29)、(7.59±0.22)、(6.48±0.21) ms;(3.75±1.88)、(4.72±2.14)、(5.43±2.29)、(5.96±2.31) μV(P=0.002、0.003、0.021、0.023和P=0.022、0.029、0.032、0.042),两组术后潜伏期明显延长且波幅降低,自1 d开始潜伏期逐渐缩短而波幅增高,两组差异有统计学意义。免疫组织化学双标染色共聚焦显微镜检测LC3阳性神经元荧光强度,假手术组为103.26±5.49,生理盐水组为121.85±8.37(P=0.002),雷帕霉素组为156.17±10.62,P=0.001;同时实时定量反转录聚合酶链反应(RT-qPCR)检测LC3和Beclin-1的mRNA相对表达量,生理盐水组为1.28±0.09和1.23�ObjectiveStudy on the effect and mechanism of rapamycin on controlled autophagy onapoptosis of spinal neurons after spinal cord injury in rats. Methods36 SD rats were randomly divided into three groups, 12 rats in each group. Sham operation group underwent laminectomy, the saline group and rapamycin group for laminectomy and use modified Allen ’s method to blow the spinal cord of T10. At postoperative 1, 3, 7, 14 d for evaluation of nervous function in rats, the level of autophagy used immunohistochemical double staining to detect and the expression of autophagy associated gene mRNA and protein microtubule-associated protein 1 light chain 3 (LC3), Beclin-1 detected at 14 d postoperative. Results1, 3, 7, 14 d the inclined plate test in saline group and rapamycin group were(28.14±2.48)°, (30.65±3.58)°, (34.85±4.12)°, (37.62±4.73)° and (29.02±2.64)°, (36.37±3.87)°, (43.28±4.39)°, (50.06±4.52)°(P=0.430, 0.021, 0.016, 0.005). Impoved Tarlov Score in saline group and rapamycin group were (0.23±0.12), (0.96±0.22), (1.17±0.28), (1.73±0.35) score and (0.29±0.15), (1.34±0.23), (2.26±0.30), (3.68±0.41) score (P=0.390, 0.037, 0.028, 0.013). Compared to sham operation group, the inclined plate test and Impoved Tarlov Score in saline group and rapamycin group were significantly decreased postoperative, then there is a certain degree of raise, the rapamycin group raised more obviously compared with saline group, there were statistical significance since the 3 d (P〈0.05). The latency and amplitude of motor evoked potentials (MEP) in saline group and rapamycin group were (17.08±0.39), (13.85±0.31), (9.26±0.23), (8.24±0.25) ms; (1.86±1.05), (2.37±1.26), (3.84±1.75), (4.27±1.49) μV and (12.63±0.34), (9.88±0.29), (7.59±0.22), (6.48±0.21) ms; (3.75±1.88), (4.72±2.14), (5.43±2.29), (5.96±2.31) μV (P=0.002, 0.003 , 0.021, 0.023 and P=0.022,
