预充式导管冲洗器与肝素钠对静脉留置针的封管效果Meta分析

摘　　要：[目的]分析预充式导管冲洗器与肝素钠对静脉留置针的封管效果.[方法]检索中国知网、万方知识服务平台和维普中文期刊资源整合服务平台中关于预充式导管冲洗器与肝素钠对静脉留置针封管效果的随机对照试验方面的文献,由2名研究者对文献质量进行评价和资料提取,采用RevMan 5.3软件进行Meta分析.[结果]共纳入12篇文献,累计研究对象1 955例.Meta分析结果显示,预充式导管冲洗器与肝素钠在静脉留置针的堵管发生率[OR=0.40,95%CI(0.18,0.89),P=0.03]、平均留置时间[SMD=1.18,95%CI(0.37,1.99),P=0.004]及局部静脉炎发生率[OR=0.36,95%CI(0.18,0.70),P=0.002]方面的差异均有统计学意义;预充式导管冲洗器与肝素钠在静脉留置针的局部外渗率方面的差异无统计学意义[OR=0.60,95%CI(0.28,1.29),P=0.19].[结论]预充式导管冲洗器对静脉留置针的封管效果在降低堵管率和局部静脉炎发生率及延长留置时间方面均优于肝素钠.OBJECTIVE To analyze the sealing effects of pre-filled catheter irrigator and heparin sodium on intravenous remaining needle.METHODS Literatures on randomized controlled trials about the sealing effects of pre-filled catheter irrigator and heparin sodium on intravenous remaining needle were retrieved through the databases such as CNKI,Wanfang data and VIP.Two researchers evaluated the quality of the literatures and extracted the data,and the RevMan 5.3 software was used to do the Meta-analysis.RESULTS12 literatures were involved,with total 1 955 objects of study.The results of Meta-analysis showed that there were significant differences between pre-filled catheter irrigator and heparin sodium in plugging rate of intravenous remaining needle[OR=0.40,95% CI（0.18,0.89）,P=0.03],the average remaining time[SMD=1.18,95%CI（0.37,1.99）,P=0.004],and the incidence local phlebitis[OR=0.36,95% CI（0.18,0.70）,P=0.002].There was no significant difference between pre-filled catheter irrigator and heparin sodium in local leakage rate of intravenous remaining needle[OR =0.60,95% CI（0.28,1.29）,P=0.19].CONCLUSION Sealing effects of the pre-filled catheter irrigator on intravenous remaining needle were better than heparin sodium in reducing the plugging rate and local phlebitis incidence and prolonging the remaining time of catheter.
