摘 要:目的探讨甲状腺乳头状癌术后^(^(131))I治疗后颈部SPECT/CT显像的价值。方法回顾性收集2015年1—12月我院PTC行甲状腺全切术后^(131)I治疗后的病例共37例,治疗后6~7天同时行^(131)I-全身显像及颈部SPECT/CT断层融合显像。结果 ^(131)I-全身显像发现颈部有放射性^(131)I浓聚灶35个,颈部SPECT/CT断层融合显像共发现47个放射性^(131)I浓聚灶,其中放射性^(131)I浓聚灶位于颈部正中甲状腺部位有33个,考虑为术后残留甲状腺;两侧颈侧区共发现4个放射性^(131)I浓聚灶;颈部Ⅰ区共发现有10个放射性^(131)I浓聚灶,考虑为转移性淋巴结摄取所致。2位患者在^(131)I-全身显像及颈部SPECT/CT融合显像上均没有发现放射性^(131)I浓聚灶。结论甲状腺乳头状癌术后^(131)I治疗后颈部SPECT/CT断层融合显像能发现更多的颈部转移性淋巴结。Objective To investigate the value of neck SPECT/CT imaging after ^131I treatment for papillary thyroid carcinoma. Methods From January to December 2015, 37 cases with PTC after total thyroidectomy by^131I in our hospital were retrospectively reviewed.^131I-whole body imaging and neck SPECT/CT tomography were performed 6 to 7 days after treatment. Results ^131I-imaging revealed 35 radioactive ^131I foci in the neck, 47 radioactive 13tI foci were found in the SPECT/CT scan of the neck, radioactive ^131I concentration loci were located in the middle part of the neck and there were 33 in the thyroid gland, which were considered as residual thyroid after operation; 4 radioactive ^131I foci were found in the bilateral neck region; 10 radioactive ^131I foci were found in the I neck region, which were considered as metastatic lymph node uptake. No radioactive ^131I concentrate was found in 2 patients on ^131I- whole body imaging and by neck SPECT/CT fusion imaging. Conclusion After the ^131I treatment of thyroid papillary carcinoma, the neck SPECT/CT tomography fusion imaging can find more neck metastatic lymph nodes.
