摘  要:目的探讨有创-无创序贯呼吸支持治疗在AECOPD合并严重呼吸衰竭的临床应用价值。方法选取2012年1月~2014年6月间我院ICU收治的AECOPD合并严重呼吸衰竭患者78例。随机分为常规机械通气组(对照组)38例和有创-无创序贯机械通气组(研究组)40例。结果研究组有创通气时间、总机械通气时间、住ICU时间、总住院时间、呼吸机相关性肺炎(VAP)发生率、再插管率及死亡率与对照组比较,显著优于对照组,相比差异有统计学意义(P<0.05)。结论有创一无创序贯性呼吸支持治疗在AECOPD合并严重呼吸衰竭中疗效确切,提高治愈好转率,优于传统有创机械通气方法。Objective To discussion the effect on patients with AECOPD and severe respiratory in invasive and Noninvasive sequential.severe respiratory failuresevere respiratory failusevere respiratory failuresevere respiratory fail.Methods From January 2012 to July 2014.78 patients with AECOPD with severe respiratory.Were randomly divided into group of 38 cases(control group) and conventional mechanical ventilation and non-invasive sequential mechanical ventilation group(research group),40cases.The control group according to conventional invasive mechanical ventilation to SIMV+PSV ventilator.Research group to invasive mechanical ventilation,when patients with pulmonary infection control window, early extubation after conscious,non-invasive double the level of positive pressure ventilation,decreasing pressure levels until out of non-invasive ventilator.Observing two groups of patients with invasive ventilation time,total time of mechanical ventilation,ICU time,total length of hospital stay, the incidence of ventilator associated pneumonia(VAP),reintubation rate and death rate.Result The research group with invasive ventilation time,total time of mechanical ventilation,ICU time,total length of hospital stay, the incidence of ventilator associated pneumonia(VAP),re-intubation rate and death rate significantly better than control group,compared with the difference was statistically significant (P〈0.05).Conclusion In invasive and Noninvasive sequential mechanical ventilation therapy AECOPD with severe respiratory failure is safe and effective treatment,improve the cure,the method is superior to the traditional invasive mechanical ventilation.
