摘  要:目的探讨综合营养护理对食管癌患者术后体重丢失及免疫功能的影响。方法选取2014年1月至2015年12月间中国医科大学附属盛京医院收治的108例食管癌患者为研究对象,根据入院先后顺序分为观察组和对照组,每组54例,其中,观察组采取围术期营养支持护理基础上联合综合营养护理,完成研究患者54例,对照组采取围术期营养支持护理,完成研究患者52例。观察并比较两组患者的体重丢失水平及免疫指标水平。结果观察组患者术后2周较就诊时体重丢失水平低于对照组患者,差异有统计学意义(P<0.05)。两组患者平时、就诊时及就诊时较平时体重丢失水平间比较,差异均无统计学意义(均P>0.05)。术后2周,两组患者的外周血淋巴细胞计数、CD4+/CD8+水平及Ig G水平均高于术前,观察组Ig G水平低于对照组,差异均有统计学意义(均P<0.05)。两组患者的TNF-α低于术前,且组间比较,观察组高于对照组,差异有统计学意义(P<0.05)。结论对食管癌患者开展综合营养护理,有助于减少患者术后体重丢失量,增强患者免疫功能。Objective To analyze the effect of comprehensive nutrition care on postoperative weight loss and immune function in patients with esophageal cancer. Methods From January 2014 to December2015,108 patients with esophageal cancer treated at Shengjing Hospital Affiliated to China Medical University were selected as the research subjects. According to the admission sequence,patients were divided into an observation group and control group with 54 patients in each group. The observation group received perioperative nutritional support combined with comprehensive nutrition care and 54 patients completed the study.The control group received perioperative nutritional support and 52 patients completed the study. Weight loss and immunity indexes were compared between the two groups. Results At 2 weeks after the surgery,weight loss was lower in the observation group than in the control group( P〈0. 05). There was no statistically significant difference in routine weight loss,weight loss during a hospital visit and the ratio between them between the two groups( P〉0. 05). At 2 weeks after the surgery,lymphocyte count in the peripheral blood,levels of CD4+/CD8+and levels of Ig G were higher than preoperatively with the observation group higher than the control group( P〈0. 05). The level of TNF-α was lower than before the surgery with the observation group lower than the control group( P〈0. 05). Conclusion Comprehensive nutrition care helps to reduce the amount of weight loss and enhance postoperative immune function in patients with esophageal cancer.
