小儿心脏手术围术期预防用抗菌药物合理性评价

摘　　要：目的分析本院小儿心脏手术围术期预防用抗菌药物合理情况。方法抽取常州市儿童医院2015年小儿心脏手术归档病例50例,对其抗菌药物的品种选择、给药时间、用法用量、用药疗程、更换药物等项目进行统计分析。结果 50例病例中,抗菌药物使用率为100%,其中合理者14例(28%)。不合理用药主要包括疗程过长、无指征更换药物、用法用量不正确等;预防用抗菌药物主要为第2代头孢菌素,其次为头霉素类。结论本院2015年小儿心脏手术围术期预防用药基本合理,但仍存在抗菌药物使用时间过长、无指征更换药物等问题,有待进一步规范。Objective To analyse the rationality of perioperative prevention with antibacterial drugs in pediatric heart surgery. Methods 50 cases of pediatric heart surgery children were selected from 2015 in Changzhou Children＇s Hospital,the antibacterial drug varieties selection,delivery time,usage and dosage,medication treatment time,changing drugs were statistical analysed. Results In 50 cases of children,antimicrobial drug utilization rate was 100%, the reasonable of 14 cases（ 28%）. Non-rational drug using mainly included the treatment for too long,no indications replacement drugs,wrong usage and dosage and so on; Prevention with antibacterial drugs mainly for the second generation of cephalosporin,followed by cephamycin. Conclusion Our hospital in 2015 pediatric heart surgery,perioperative prophylaxis is basically rational,but there are antibacterial drug using time is too long,no indications replacement drug problems,so needs to be further standardized.
