外固定支架治疗老年性骨远端C型骨折的价值

摘　　要：目的:探讨外固定支架治疗老年性桡骨远端C型骨折的效果。方法:选取192例老年性桡骨远端C型骨折患者,随机分为观察组与对照组各96例。观察组采取外固定支架治疗,对照组采取掌侧锁定钢板内固定治疗,比较两组术前及术后2个月的尺偏角、掌倾角与桡骨高度、住院时间与骨折愈合时间、并发症总发生率及术后1年的腕关节优良率。结果:术后2个月两组尺偏角、掌倾角与桡骨高度均较术前明显恢复,前后差异有统计学意义(P<0.05),组间差异无统计学意义(P>0.05);观察组住院时间与骨折愈合时间分别为(6.6±1.6)d、(3.4±0.6)个月,均短于对照组(P<0.05);观察组并发症总发生率8.4%,对照组为12.6%(P>0.05)。观察组术后1年腕关节优良率83.3%,对照组为87.5%(P>0.05)。结论:外固定支架治疗老年性桡骨远端C型骨折的近期腕关节优良率与术后并发症发生率与钢板内固定相当,同时骨折愈合快,住院时间缩短,有一定优势。Objective To explore the effect of external fixation in treatment of senile radial distal C-type fracture. Methods Total 192 senile patients with radial distal C-type fracture were randomly divided into ob- servation group and control group, 96 cases in each group. The observation group received external fixation treat- ment, while the control group received palm side locking plate internal fixation treatment. The ulnar angle, palm inclination, radial height, hospitalization time, fracture healing time and total incidence of complication were compared between the two groups before and two months after operation. The excellent rates of wrist for one year were also compared between the two groups. Results The ulnar angle, palm inclination and radial height in two groups after 2 months of operation were significantly restored than those before operation （P 〈 0.05）. Howev- er, the difference between the two groups was not significant （P 〉 0.05）. The hospitalization time and fracture healing time were （6.6±1.6） days and （3.4±0.6） months in observation group, which were shorter than those in control group （P 〈 0.05）. The overall incidence of complication in the observation group was 8.4% and that in the control group was 12.6% （P 〉 0.05）. The excellent rates of wrist joint were 83.3% in the observation group and 87.5% in the control group （P 〉 0.05）. Conclusion The recent excellent rate of wrist and the incidence of postoperative complication of external fixation stent in the treatment of senile radial distal C-type fracture were equivalent to the plate internal fixation, with faster fracture healing, shorter hospital stay, and certain advantages.
