摘  要:目的 系统评价计算机辅助认知训练改善痴呆症患者认知障碍的效果.方法 采用Meta分析的方法,通过计算机检索PubMed、EMbase、Web of Science、Cochrane Library、CNKI、VIP、CBM、WanFang Data数据库,检索时限为建库至2016年12月,搜集与计算机辅助认知训练和痴呆症患者认知功能相关的随机对照试验.由两名研究者独立按照纳入排除标准提取资料并进行文献质量评价后,采用RevMan 5.3进行分析.结果 共纳入9篇文献,总样本量354例,平均年龄(75.4±5.24)岁,文献总体质量中等.Meta分析显示,CCT干预组痴呆患者总体认知功能得分较对照组高(WMD=0.36,95%CI:0.13~0.58,P=0.002),记忆功能也较对照组高(WMD=0.37,95%CI:0.13~0.61,P=0.002).亚组分析显示,CCT对不同程度痴呆患者总体认知功能均有提升作用,血管性痴呆CCT组总体认知功能改善虽然高于对照组,但差异无统计学意义(P〉0.05).CCT对轻度痴呆患者的记忆功能的作用与对照组相比,差异无统计学意义(P〉0.05).结论 CCT可以改善阿尔茨海默病患者的总体认知功能和记忆功能,但对血管性痴呆患者的认知功能以及轻度痴呆患者记忆功能是否有效有待进一步研究.Objective To evaluate the effect of computerized cognitive training on improving the cognitive impairment in patients with dementia.Methods Literature review was conducted according to the database including PubMed, EMbase, Web of Science, Cochrane Library, CNKI, VIP, CBM and WanFang Data. The research of random control trials (RCTs) that related to CCT and cognitive function published before December 2016 was collected. According to the inclusive and exclusive criteria, two researchers were responsible for extracting data and evaluating the quality of data separately. The meta-analysis was conducted using RevMan 5.3 software.Results A total of nine RCTs were included. The total sample size was 354 and the average age was (75.4±5.24) years old. The literature showed medium quality of data. The meta-analysis showed that the cognitive function scores in patients with CCT were significantly higher than those without CCT intervention (WMD=0.36, 95%CI:0.13-0.58,P=0.002). Patients with CCT also showed significantly better memory function compared with patients without CCT (WMD=0.37, 95%CI: 0.13-0.61, P=0.002). Subgroup analysis showed that CCT could improve the general cognitive function in patients with different degree of dementia but no significant effect was found in patients with vascular dementia (P〉0.05). There were no statistically significant effects of CCT on memory function in patients with mild dementia (P〉0.05).Conclusions CCT can improve the general cognitive function and memory function in patients with Alzheimer's disease. However, the effect of CCT on cognitive function in patients with vascular dementia and the effect on memory function in patients with mild dementia should be further studied.
